Configure Measures for Claims-Based Reporting

On the Claims Based Reporting Setup panel, system administrators can add existing or newly-created measures to practices. Open NextGen® Enterprise EHR. On the Provider CQM Maintenance template, expand the Claims Based Reporting Setup panel. Claims Based Reporting Setup panel In the Practice(s) to View/Update section, do one of the following: To view or update measures for all practices, select the All option. To view or update measures for a practice, select the Selected option, and select a practice from the list. In the System field, do one of the following: To add an existing measure, select QID, select the Measure # field, and select a measure on the Dbp Provider Measure List window. Dbp Provider Measure List To add new claims-based measure, select Claims Based Measures. Select the Measure # field, and select <Add New> on the Dbp Provider Measure List window. Configure Non-CMS Provider Measures

You can configure non-CMS Provider measures for CPT II codes. To enable a provider to document an exception and submit a CPT II code manually, you must assign a measure to a provider. Open NextGen® Enterprise EHR. Open the Provider CQM Maintenance template. Provider Measures (not CMS) Panel In the Provider Measures (not CMS) panel, select the Provider field. Select a provider in the list. Select the System field, and select Claims Based Measures. Note: The grid displays measures that have been set up in the Claims Based Reporting Setup panel on the Provider CQM Maintenance template that matches the System selection you have selected. Select the Measure # field, and select a measure in the list.The Measure short description field displays the description of the measure. Select Add.The Provider Measures (not CMS) configuration is added to the All Providers Measure grid. Update Non-CMS Provider Measures You can update the non-CMS Provider measures that are configured for the CPT II codes.

Submit CPT® II Procedure Codes and Measure Exceptions

You can manually submit CPT II procedure codes and the measure exceptions in the Procedure Coded Measures panel on the Exceptions/Measures template. Open NextGen® Enterprise EHR. To open the Exceptions/Measures template, do one of the following: From the navigation bar, select Exceptions/Measures. From the Finalize template, expand the CPT II Claims Based Reporting panel, and select the Exceptions/Measures link. Select the Exceptions/Measures link on templates. Expand the Procedure Coded Measures panel. Procedure Coded Measures Select a measure from the grid.The fields below the grid are filled accordingly and the I accept responsibility to provide supporting documentation checkbox is selected. Note: Measures that have modifiers indicate the measure is an exception. Document Registry-Based Exceptions

You can document a registry based exception. Open NextGen® Enterprise EHR. To open the Exception/Measures template, do one of the following: In the navigation bar, select the Exceptions/Measures link. On the Finalize template in the CPT II Claims Based Measure Reporting panel, select the Exceptions/Measures link. Exceptions/Measures For Display, select one of the following: Provider Measures Exceptions (current template) Exceptions (all) Select a measure in the grid.The details of the selected measure appear in the Measure Details and Exception Management sections. If applicable, select the exception class for the measure. Note: If the measure exists in multiple exception class records, a window appears, where you can select the exception class. Reporting Through Custom Data Mapping

You can use the Custom Data Mapping panel for reporting. When you update NextGen® Enterprise EHR, data recorded on the Custom Data Mapping panel can appear on other templates if configured. The Custom Data Mapping panel on the Exceptions/Measures template includes measures that are configured in the Certification Custom Data Mapping panel on the Coded Result Mapping template. Submitting Procedure Coded Measures

You can select and submit a procedure code to satisfy a claims-based measure. The Procedure Coded Measures panel on the Exceptions/Measures template displays a grid with configurable Quality measures (identified by QID) and HEDIS measures from the Provider CQM Maintenance template and Claims Based Reporting Setup panel. Measures that are excluded from claims-based reporting must be reported on the Exception/Measures template. You can submit exceptions for claims-based measures in the Procedure Coded Measures panel. The measures that display modifiers in the Modifier column are exceptions. Procedure Coded Measures Submit CPT II Procedure Codes and Measure Exceptions You can manually submit CPT II procedure codes and the measure exceptions in the Procedure Coded Measures panel on the Exceptions/Measures template.
