Document Pain Assessment and Plan of Care

You can document the pain assessment details and follow-up plan of care. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel, and then select the Health Promotion Plan link.The Health Promotion Plan template opens. Expand the Pain Management panel. Note: The Pain level and Method fields display the details entered on the Vital Signs template. You cannot update the pain level and method in the Pain Management panel. Pain Management - Health Promotion Plan To manage exceptions and view measure details on the Exceptions/Measures template, select Exceptions. If details are not entered in the Pain level section on the Vital Signs template, select the following fields, and select the appropriate option in the list. Pain level Method Duration Quality To indicate to see the HPI details, select the See HPI checkbox. Enter the location of the pain of the patient. Document Health Promotion Plans

You can document the Health Promotion Plans such as BMI plan, depression plan, and hypertension plan of a patient. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel and then select the Health Promotion Plan link.The Health Promotion Plan template opens. Expand the Health Promotion Plan panel. Add a diagnosis. Select Plan and select a plan from the list.Based on the selected plan, the different sets of fields appear. To add the plan details, do the following: For BMI Plan, add the diet, physical activity, and referrals details. For Depression Plan, dd the follow-up and referrals details. For Hypertension Plan, add the diet, physical activity, referrals, and lifestyle details. Note: To turn on the Referrals section and order a referral, you must add a diagnosis. To indicate that the referral is internal, select the Internal referral checkbox. If required, in the Details field, enter any additional information. If you want to add or update patient educat

Screen for Future Fall Risk

You can document the patient's risk for falls. Open NextGen® Enterprise EHR. To open the Social History – Lifestyle template, do one of the following: On the Histories template, on the Social panel, select Lifestyle , and select Add/Edit. In the History Summary panel on Intake, select Lifestyle from Social, and select Add/Edit. The Social History – Lifestyle template opens. In the Home Environment/Safety section, in Falls in the last year?, select the applicable option. If you select Yes, enter the number of falls. In Did the fall(s) result in injury?, select the applicable option. If you select Yes, then enter information about any injuries resulting from falls in the Details field. In Is the patient at risk for falls?, select the applicable option. Select the Fall Risk Plan link to access the Fall Risk section of the Health Promotion Plan template, if needed. Document the Patient's Suicide Risk Assessment

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Eval of Suicide Risk (BH), and then select OK. Select the program, specialty, and visit type. Note: Only programs that the patient is actively enrolled in appear in the list. You can select <No Program> to create an encounter without a program, and then add the program later. Document the reason for the suicide risk assessment. Document the patient's historical, clinical, and situational risk factors. Document the protective factors for the patient's mental health. Document Patient Activities to Ensure a Safe Environment for the Patient

In NextGen® Enterprise EHR, in Patient History, select the Templates button. The Select Template window opens. On the Medical Records tab, select All. From the list, select Patient Safety Plan (BH) and select OK. Expand the Safe Environment panel, and enter a brief description of the patient's activities that contribute to maintaining a safe environment. To indicate whether the activities are currently relevant, select Active or Inactive. Select Add. The safety activity record appears in the grid. Optional: To filter the list of records in the grid, select the status in the Display status of field. The grid lists records with the selected status. Document the Patient's Height

You can document the height of a patient in inches or in feet using the Vital Signs template. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. Expand the Vital Signs panel. Select Add.The age-specific vital signs template opens. In Height/length measurements, do one of the following: To enter the current day's data, fill the ft, in, or cm fields within the 1 – 9.99 ft, 12 – 119.88 in, or 30.48 – 304.49 cm range, respectively. To enter previously documented data, select the Carried forward option. If you enter the height in either the ft, cm, or in field, the NextGen® Enterprise EHR fills values in the other fields automatically. Note: A message appears if the value entered is not within the range. Indicate the position of the patient when the height is measured.
