Edit an Encounter Assessment
Open NextGen® Enterprise EHR. On the Subjective template, General panel, Encounter Diagnosis section, highlight an assessment within the grid. Click Edit to open the Assessments template. Make any necessary changes and then click Add/Update. Click Save & Close. Parent topic: Encounter Diagnosis

Add an Encounter Diagnosis
Open NextGen® Enterprise EHR. On the Subjective template, go to the General panel and click Add under the Encounter Diagnoses section. Click the Add common assessment link to view the common assessments for Physical Therapy, and do one of the following actions: If the required assessment is available on the Common Assessment window, click the assessment link, select an appropriate description from the resulting list, click OK, and then click Save & Close. If the required assessment is not available on the Common Assessment window, click the Diagnosis Code Lookup link, search for and select a diagnosis, and then click Select. Select the Show Historical assessments checkbox to display the patient's historical assessments in the grid. Parent topic: Encounter Diagnosis

Encounter Diagnosis
For every patient encounter, use the Encounter Diagnoses section to define all the assessments to be included in orders and billing and to view billing assessments for the current encounter, including ICD codes. The Encounter Diagnoses section appears on multiple templates throughout the Physical Therapy workflow and maintains the same functionality. Remove a Treating Assessment
Open NextGen® Enterprise EHR. On the Subjective template, General panel, Treating Assessments section, highlight the assessment to be deleted in the grid. Click Remove. Parent topic: Treating Assessments

Assessments Panel
Encounter Diagnoses and Treating Assessments can be reviewed and updated on the Assessments panel on the PT Finalize template. The Assessments panel functions the same way as the Diagnosis panel on the Home Page and the Treatment Log templates. The Encounter Diagnoses grid displays current encounter diagnoses. The Treating Assessment grid may contain the same diagnoses as the Encounter Diagnoses grid or they may be different. Delete a Diagnosis
You can delete a diagnosis that was added on an encounter. Before you begin Make sure that the applicable template with the Diagnosis grid is open. In the Diagnosis grid, select the diagnosis. Diagnosis grid Click Remove. Note: If you are not on the encounter where the diagnosis was added, a message appears stating that the diagnosis can be removed only from the encounter where it was added. The Reason window opens.
