Edit a Diagnosis

You can edit the description of a diagnosis. Before you begin Make sure that the applicable template with the Diagnosis grid is open. In the Diagnosis grid, select the diagnosis. Diagnosis grid Click Edit. The IORTS Dx Edit window opens. IORTS Diagnosis Edit window Click the Diagnosis field, and edit the description. Click Update to update the entry and close the window. Remove Encounter Diagnoses from an Encounter to be Deleted

Access the Patient Chart. Select the Encounters tab, and then the General sub-tab. In the ICD-CM section, select the Open Record button. The Diagnosis Selection window appears. From the Selected ICD-CM Codes section, select a diagnosis code to be deleted. Select the blue left arrow button to remove the code from the encounter. A prompt appears asking to confirm the removal of this diagnosis. To remove the code, select OK. One of the following occurs: If the diagnosis code is not associated with charges, it is removed from the encounter. If the diagnosis code is associated with a charge, then a prompt appears stating that this diagnosis code is used on a charge and that it cannot be removed. Repeat steps 4-6 for additional diagnosis codes to be deleted. To save changes and close the window, select OK. Parent topic: Delete Encounters

Remove Claims from an Encounter to be Deleted

Claims created for an encounter must be deleted before the encounter can be deleted. Access the Patient Chart. Select the Encounters tab, and then the General sub-tab. From the Encounters list, select the encounter to be deleted. Right-click and select Claims from the menu. The Encounter Claim Requests window opens. Select a claim to be deleted. Right-click and select Delete from the menu. A prompt appears asking if you want to delete this claim and all associated claim requests. To delete the claim, select Yes. The claim is deleted from the encounter. Repeat steps 5-7 to delete additional claims. To close the Encounter Claim Requests window, select Close. Parent topic: Delete Encounters

Remove a Diagnosis Code from a Diagnosis Group

You can remove a diagnosis code from an assigned diagnosis group. On the Specialty Diagnosis Group Config practice template, expand the Manage Diagnosis Groups and Codes panel. From the Add Diagnosis Group grid, select the desired record. All the assigned codes to selected group appear in the Assigned Diagnosis Codes grid. Select the desired code. The X appears after the selected code. Use the left-pointing arrow to remove the selected code from the grid. Click Update. Parent topic: Managing Diagnosis Groups and Codes

Manage Historical Diagnostic Study

To open the Diagnostic History Entry template, do one of the following: In the History Summary panel, select Diagnostic, and click Add/Edit. On the Histories template, in the Diagnostic Studies panel, click Add. The Diagnostic History Entry template opens. To update the historical diagnostic study, do the following: On the Diagnostic History Entry template, select a historical diagnostic study that need to be updated from the grid. Update the Interpretation and / or Result as needed. Click Update. The record is updated. If the record was previously added to the Reason for Visit panel, the corresponding record in also updated in the Reason for Visit panel. Close the template. To delete a historical diagnostic study, do the following: On the Diagnostic History Entry template, select a historical diagnostic study that need to be deleted from the grid. Click Delete. Delete an Existing Assessment

CAUTION: Removing an item from the Today's Assessment grid does not resolve the diagnosis. The Add or Update Assessments template is part of a group of templates that use the last assessment or diagnosis selected on any of the templates as the working assessment. If you remove one of multiple available assessments on the Add or Update Assessments template, you must select a new working assessment that carries over to the other templates that share the same working assessment. For more information, see Shared Working Assessment On the Assessments tab , in the Today's Assessment grid, select the row to be deleted. Click Remove. Click Save & Close.
