Add an Encounter Diagnosis

Open NextGen® Enterprise EHR. On the Subjective template, go to the General panel and click Add under the Encounter Diagnoses section. Click the Add common assessment link to view the common assessments for Physical Therapy, and do one of the following actions: If the required assessment is available on the Common Assessment window, click the assessment link, select an appropriate description from the resulting list, click OK, and then click Save & Close. If the required assessment is not available on the Common Assessment window, click the Diagnosis Code Lookup link, search for and select a diagnosis, and then click Select. Select the Show Historical assessments checkbox to display the patient's historical assessments in the grid. Parent topic: Encounter Diagnosis

Remove an Encounter Assessment

On the Subjective template, General panel, Encounter Diagnosis section, highlight the assessment to be deleted. Click Remove. Parent topic: Encounter Diagnosis

Edit a Treating Assessment

Open NextGen® Enterprise EHR. On the Subjective template, General panel, Treating Assessments section, select the assessment in the grid to be updated. Click Edit to launch the PT Treating Assessment template. Select the item to be updated in the Treating assessment grid. Make any updates or changes as needed. Open NextGen® Enterprise EHR. On the Subjective template, General panel, Treating Assessment section, click Add.The PT Treating Assessment template opens. PT Treating Assessment Do one of the following: Click the Add Common Assessment link, click an appropriate assessment link, select a description from the resulting list, and then click OK. Add the Diagnosis for Therapy Evaluation

Open NextGen® Enterprise EHR. Navigate to the Evaluation panel on the PT Finalize template. To associate the diagnosis with Therapy Evaluation: Highlight an assessment displayed in the Encounter Diagnoses grid. Click the Dx code field.The selected assessment’s Dx code appears in the field. To clear out all the fields in the Evaluation section, click Clear. If applicable, re-enter evaluation details by first selecting the Therapy Evaluation visit type. Parent topic: Evaluation Panel
