Evaluation Panel

The Evaluation panel on the PT Finalize template is where you can make changes to the evaluation visit type and its associated code, add up to three modifiers, enter a number of billing units, and select a diagnosis to be associated with the evaluation visit. Finalize Template

You can use the Finalize template at the end of an encounter to review the encounter, record and calculate time-based and service-based Current Procedural Terminology (CPT) codes, and associate the appropriate diagnosis codes for billing. This template contains Assessments, Payer, Evaluation, Procedures, and Wound Care panels. Treating Assessments

The grid in the Treating Assessment section enables you to add assessments for therapy treatments. The treating assessments may be the same as the encounter or ordering diagnoses, or they may be more general. For example, the treating diagnosis may be pain hip/pelvic with one diagnosis code, while the encounter diagnosis or ordering assessment may be sprain hip/thigh with a different diagnosis code. The Treating Assessment section appears on multiple templates throughout the Physical Therapy workflow and maintains the same functionality. Encounter Diagnosis

For every patient encounter, use the Encounter Diagnoses section to define all the assessments to be included in orders and billing and to view billing assessments for the current encounter, including ICD codes. The Encounter Diagnoses section appears on multiple templates throughout the Physical Therapy workflow and maintains the same functionality. Review Today's Assessments

Ophthalmic providers can review previously documented diagnoses/assessments that were selected for the patient for this encounter listed in the Today's Assessments grid in the Diagnosis panel on the Finalize - OPH template. CAUTION: A diagnosis added or modified on the Finalize - OPH template does not appear in the Impression/Plan Summary grid. However, it appears in the Diagnosis History and in the Chart Note. Add the Diagnosis for Therapy Evaluation

Open NextGen® Enterprise EHR. Navigate to the Evaluation panel on the PT Finalize template. To associate the diagnosis with Therapy Evaluation: Highlight an assessment displayed in the Encounter Diagnoses grid. Click the Dx code field.The selected assessment’s Dx code appears in the field. To clear out all the fields in the Evaluation section, click Clear. If applicable, re-enter evaluation details by first selecting the Therapy Evaluation visit type. Parent topic: Evaluation Panel
