and enter the information on the

Add Patient Demographic Information
You can add demographic details of a patient on the Patient Demographics template. Open NextGen® Enterprise EHR. Open the Patient Demographics template, and then expand the Patient Information panel. Patient Information Enter values for the following fields: State of birth Country of birth Blood type Enter information in the Race and Ethnicity fields. Note: You can add multiple values for race and ethnicity. To enter the patient's social security number, preferred language, and marital status on the Modify Patient Information template, select the corresponding fields.The Modify Patient Information window opens. The social security, preferred language, and marital status information you update in the Modify Patient Information window automatically appears in the Patient Information panel of the Patient Demographics template. View and Update Patient Demographic Information
You can access the Patient Demographics template from Demographics links on various NextGen® Adaptive Content Engine templates. For more information, go to the NextGen Healthcare Success Community, and download the NextGen® Adaptive Content Engine documentation for the specific template. Open NextGen® Enterprise EHR. Open an existing patient record. On the main toolbar, select Template . In the Select Template window, select the Demographics tab. Select the Patient Demographics template. Expand the panels to view the information. Do one of the following: To save your updates without closing the template, select File > Save. Viewing Patient Demographics Information
The Demographics tab in Patient Information is the default tab that appears on the patient chart displaying demographics, patient status and client-defined information for the patient. You can select Who/When button to view the Created By and Last Modified By information. The demographics information consists of gender identity, preferred pronoun, current gender, sexual orientation, veteran status, and pharmacy information. The fields that appear for demographic information depend on the selections made on the Enterprise Preferences Client Defined tab and/or the Practice Preferences of the Chart window. You can create the Patient Expired label on an expired patient's chart by selecting the Expired check box and entering the expired date on the Demographics tab in the Add Person Information or Modify Person Information window For more information on adding or modifying patient demographic information, go to NextGen Healthcare Success Community and download the Patient Demographics Guide

You can also access the Patient Demographics template from Demographics links on various NextGen® Adaptive Content Engine. For more information, go to the NextGen Healthcare Success Community, and download the NextGen® Adaptive Content Engine documentation for the specific template. Open an existing patient in NextGen® Enterprise EHR. For more information, refer to the Person and Patient Lookup in EHR section. Open the Select Template window and select the Demographics tab. Select the Patient Demographics template. For more information on selecting a template, go to the NextGen Healthcare Success Community and download the Templates User Guide for NextGen Enterprise EHR. The Patient Demographics template appears, displaying the selected patient's information. Expand the panels to view the information in them. You can add or edit information in the enabled fields. Patient Demographic Template
The Patient Demographic Template section allows you to configure the following: Patient Checked In options - If you select email, tasking, or pt. tracking option, Patient Checked In on the Patient Demographics template opens the corresponding template. For example, if you select pt. tracking option, the system opens the Patient Tracking template from Patient Checked In. The do not use, hide btn option hides Patient Checked In on the Patient Demographics template. Patient Demographics template Set fields to Read-Only - If you select the Make fields on patient template read-only checkbox, the patient and person fields such as name and contact information appear unavailable to edit on the run-time templates. However, values in the patient and person fields can be edited in the Patient Information panel on the Patient Demographics template of the chart.
