Patient's General Information

You can use the General panel to enter a patient's general information. You can document a patient's general information on the Intake template. General Information Section on Intake When someone provides the patient details, you can document the historian details in the Historian field. When someone interprets the patient’s communication, you can select the Interpreter Details link to document the details of the interpreter. You can select Configurable Questionnaires link to document the screening details of the patient. You can select the Transitioning into Care link to document the summary of care record information for a patient. You can select the Enable HCC for this patient check box to hide or show the HCC links, badges, and grid columns on the templates at the patient level. You can select the Family planning encounter and Disability check boxes if an encounter is a family planning encounter and describes any disability. Obstetrics Histories Template

You can document information in the OB Histories template. The Histories template contains nine panels: Pregnancy History Gravidity/Parity Since Last Menstrual Period Medical/Infection History Medical/Surgical/Interim General Family Social Genetic Screening/Teratology Counseling The Histories template displays gravidity/parity, initial/working EDD, working EGA, blood type and Rh, and risk level as in Gestational Age template. After a complete history is taken, use the Risk Factors link to document the risk level. Note: When documenting risk factors, the practice may decide to only indicate the positive items rather than select negative for all of the remaining items. In addition, putting the actual risk level must be done by the provider. This may be done on this template or on the Prenatal Detail template. Gestational Age Template

You can document the gestational age to determine the Estimated Date of Delivery (EDD). The Gestational Age panel allows you to document the necessary information about gestational age so that it is possible to determine the Estimated Date of Delivery (EDD). Note: NextGen® Adaptive Content Engine calculates the EGA (Estimated Gestational Age) based on the LMP date provided in the Gestational Age template . The gestational age for each encounter recalculates automatically and fills in the appropriate templates. The system recalculates the gestational age after each encounter and fills in the appropriate template. Note: The OB Gestational Orders template is integrated in the NextGen® Adaptive Content Engine workflow. You can open the OB Gestational Orders template from Prenatal Detail > Order Labs > OBGYN Lab Master > Labs > Orders by Gestational Week. Document the Obstetric Education

You can document the first, second, and third trimester information in the Education template. The Education template allows you to document information related to prenatal education for each trimester. One or multiple people can provide details. There are three panels on the Education template, one for each trimester of pregnancy. In addition, the Ultrasound Detail link opens a template on which you can document in-office official sonogram results, if needed. Open the Education-Obstetrics template. Open the required trimester edcation panel, for example, the First Trimester Education panel. Select the Add/Update button.The First Trimester window opens. Select the Check all education for 1st Trimester checkbox or select the relevant checkboxes indicating that the education element is discussed with the patient.The following fields are automatically filled: Completed fields with the current encounter date. Obstetrics Intake Staff Workflow

You can document the intake information in the Intake template. The intake staff workflow for an Initial Prenatal visit involves documenting the patient encounter on the Intake, Gestational Age, Histories, and Education templates. The information documented by the intake staff is reviewed by the provider. When you open the Intake template, select Obstetrics and OB Prenatal Initial from the Specialty and Visit Type fields to display the Initial Prenatal template set. The Intake template includes various links to add more information about the patient. Review of Obstetrics Details by Intake Staff The intake staff can review the patient's details documented during the previous encounters. Viewing Links on the Intake Template The links on the Intake template allows you to open the templates where you can document the patient details. Documenting Obstetrics Gestational Orders

You can document information in the OB Gestational Orders template. You can open the OB Gestational Orders template from thePrenatal Detail > Order Labs > OBGYN Lab Master > Labs > Orders by Gestational Week. The lab results fills the Order Management template, Lab Results - Obstetrics template, the Order module, and the master document. Before using the OB Gestational Orders template, you have to select the OB Prenatal- Initial visit type and enter the applicable information in the Gestational Age tab.
