Histories Tab Links

You can document information in the Histories tab links. The Histories template includes the following links: Demographics - The Demographics link displays the Patient Demographics template to allows you to document patient information such as contact information, address details, Primary Care Physician (PCP) details, insurance or pharmacy details, provider details, emergency contact details, reminders, and employment details. Order Management - The Order Management link displays the Order Management template, which allows you to update the applicable order details. Document Library - The Document Library link opens the Documents and Letters template. The Documents and Letters template includes links that generate the formatted letters and assessment templates, allows you to enter the applicable data. Documenting Obstetric Patient Histories

You can document information in the OB Histories template. To document information in the OB Histories template, select the Histories tab. By default, all panels on this template are expanded when the template is opened. Document the Pregnancy History You can document pregnancy history information in the Pregnancy History panel. Document the Gravidity and Parity You can quickly view the patient's gravidity and parity and can only document multiple births information in the Gravidity/Parity panel. Document the Last Menstrual Period You can document last menstrual period information in the Since Last Menstrual Period panel. Document the Medical and Infection History You can document the medical infection history information in the Medical/Infection History panel. Gestational Age Template

You can document the gestational age to determine the Estimated Date of Delivery (EDD). The Gestational Age panel allows you to document the necessary information about gestational age so that it is possible to determine the Estimated Date of Delivery (EDD). Note: NextGen® Adaptive Content Engine calculates the EGA (Estimated Gestational Age) based on the LMP date provided in the Gestational Age template . The gestational age for each encounter recalculates automatically and fills in the appropriate templates. The system recalculates the gestational age after each encounter and fills in the appropriate template. Note: The OB Gestational Orders template is integrated in the NextGen® Adaptive Content Engine workflow. You can open the OB Gestational Orders template from Prenatal Detail > Order Labs > OBGYN Lab Master > Labs > Orders by Gestational Week. Obstetrics Intake Staff Workflow

You can document the intake information in the Intake template. The intake staff workflow for an Initial Prenatal visit involves documenting the patient encounter on the Intake, Gestational Age, Histories, and Education templates. The information documented by the intake staff is reviewed by the provider. When you open the Intake template, select Obstetrics and OB Prenatal Initial from the Specialty and Visit Type fields to display the Initial Prenatal template set. The Intake template includes various links to add more information about the patient. Review of Obstetrics Details by Intake Staff The intake staff can review the patient's details documented during the previous encounters. Viewing Links on the Intake Template The links on the Intake template allows you to open the templates where you can document the patient details. Obstetrics Patient Header

You can view the Obstetrics fields in the Obstetrics (OB) patient header. You can view the following data in the OB Patient Header: Gravidity, Parity (G/P/T/P/A/L): Filled from the Histories template. Initial/Working EDD: Filled from the Gestational Age template. Working EGA: Filled from the Gestational Age template. Blood Type and Rh: Filled from the Lab Orders template. Risk Level: Filled from the OB Risk Factors template. Document the Pregnancy History

You can document pregnancy history information in the Pregnancy History panel. You can use the Pregnancy History panel to open the Pregnancy Outcomes template. Note: The Pregnancy Outcomes template is used during Prenatal and Postpartum visits as well as gynecology encounters. The workflow is different for each. In general, the most detail is entered during a Postpartum Visit. Pregnancy History Open the Histories tab. Open the Pregnancy History panel. Select the Add/Update button in the Pregnancy History panel.The Pregnancy Outcomes template opens. If you turn on the Parity auto-calculation setting, the Auto-calculation green text appears.
