Document the Gravidity and Parity

You can quickly view the patient's gravidity and parity and can only document multiple births information in the Gravidity/Parity panel. Before you begin You must enter the gravidity and parity information in the Pregnancy History panel. The Gravidity/Parity panel allows you to view the gravidity and parity information and enter multiple births. Open the Histories template. Expand the Gravidity/Parity panel. Note: You can only view the details in the Gravidity/Parity panel. Optional: To enter multiple births, enter the number in the Multiple births window.The multiple births number displays. Document the Medical, Surgical, and Interim Histories

You can document the medical infection history information in the Medical/Surgical/Interim panel. You can filter the past history records in the grid by selecting the Medical History/Surgical History/Interim History checkboxes. By default, Show All checkbox is selected. Open the Histories tab. Open the Medical/Surgical/Interim panel. Select Add to open the Past Medical History template. Select the checkbox corresponding to the correct entry. Note: Items in blue are categories rather than individual selections. Selecting the blue name, or selecting the checkbox opens a list of choices. The Add Pregnancy Detail button in the Medical panel opens the Pregnancy Outcomes template. Select the Add to Grid button. Past Medical History Expand Surgical to enter past procedures/operations. Document the Pregnancy Outcomes Template

You can document information in the Pregnancy Outcomes template. The Pregnancy Outcomes template provides you the ability to view and modify a patient’s gravidity and parity. You can view and modify the total parity values with or without the details of each pregnancy. Minimal required fields are Pregnancy Entry Type, Pregnancy #, and Delivery Type. It is preferable, on an OB initial visit to use the gravidity/parity fields and Pregnancy Outcomes templates from within the OB templates. When you activate parity auto-calculation, parity automatically calculates the details when you document historical pregnancy or end a current pregnancy When you select End current pregnancy the Gravidity reflects a non-pregnant status. This option is used on a postpartum or post-abortion visit and not on an initial prenatal visit. On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN.The OBGYN Synopsis template opens. Select the Add/Update Detail button in the OBGYN Synopsis template.

Update the Pregnancy History

You can update pregnancy history information in the Pregnancy History panel. Open the Pregnancy History panel. Highlight an entry in the Pregnancy History grid. Select Add/Update.The Pregnancy Outcomes template opens. Highlight an entry in the Pregnancy history grid. Update the required information. Select Update to add the edited information to the grid. Documenting Obstetric Patient Histories

You can document information in the OB Histories template. To document information in the OB Histories template, select the Histories tab. By default, all panels on this template are expanded when the template is opened. Document the Pregnancy History You can document pregnancy history information in the Pregnancy History panel. Document Pregnancy Details

You can document the pregnancy details for female patients as a part of medical history. On the Past Medical History template, in the Medical panel, click Add Pregnancy Detail. The Pregnancy Outcomes template opens. Pregnancy Outcomes template In the Quick Entry/Edit panel, in the Parity, Outcome Details, and Other sections, click the appropriate fields, and enter the values. In the Detailed Entry/Edit panel, enter the details of the pregnancy. Note: The fields that are marked with asterisk (*) are mandatory. Click Add To Grid to add the documented details to the Pregnancy history grid. Click Save & Close.
