Histories Template

The Histories template enables you to document a patient's social history, problem list, medical or surgical history, diagnostic study history, and family history. The panels available in the Histories template are as follows: Table 1. Section Description Problem List You can view, add, modify and retrieve patient's clinical problems and chronic conditions. Medical/Surgical/Interim You can document the patient's problem list and social, medical or surgical, diagnostic study, and family histories. Diagnostics Studies You can document the patient's diagnostic tests and its associated results. Family You can document the family history of the patient. Social You can document the social history of an adult or a pediatric patient. The categories in the social history includes tobacco/vaping use, alcohol/caffeine, lifestyle, occupation and so on. Document Dental Care Details of Pediatric Patients

You can document dental care for a pediatric patient in the Social panel on the Histories template. The options available to document dental care for a pediatric patient change based on the pediatric age group. These templates enable you to document age-appropriate nutrition information for pediatric patients. Open NextGen® Enterprise EHR. Open the Histories template. Expand the Social panel and select Nutrition. Click Add/Edit. Based on the patient's age, one of the following windows opens: Interval History - Nutrition birth- 12 mo for 0 to 12 months Interval History - Nutrition 1 to 4 yr for 1 to 4 years Interval History Nutrition Over 5 yr for 5 years or older To review the existing CQM – Dental Caries details of the patient, click the quality icon. Add Social History

Before you begin You can configure the show confidential history, panel version of adult and pediatric social history templates from the Social History section of the Practice Configuration template. For more information about these settings, go to NextGen Healthcare Success Community and download the Practice Configuration Template User Guide for NextGen® Adaptive Content Engine You can add social history information in the History Summary section. The Social section uses age-specific templates based on the patient age and the Pediatric Age configuration on the Practice Configuration practice template. Open NextGen® Enterprise EHR. Open the Intake template. In the History Summary section, select Social. Select Select Social Hx to Add/Edit and select the relevant option from the list.Based on the patient age, the relevant social history template opens. Document the reported history information. Pediatric Confidential Information

You can use the confidential history templates to document a patient's confidential details. You can open the Peds Confidential History template by clicking the following links: The Confidential History link in the Social panel on the Histories template. The Confidential link in the History Summary panel on the Intake template. You can document the following confidential details of the pediatric patient: Alcohol and drug use, and any related comments from the historian on the Peds Confidential History - Substances template. Psychiatric history, child placement history, and incarceration details from the historian on the Peds Confidential History - Psychiatric template. Sexual practices, Sexually Transmitted Infection (STI) details, and any related comments from the historian or the provider, or from both on the Peds Confidential History - Sexual Hx template. Patient's Adult Social History

You can view and document the social history of an adult patient. You can document the social history information of the patient in a one-page panel format on the Adult Social History template. This template does not appear by default. The Social History Review section allows you to document the review status. You can select an option to complete the social history review. You can select the Enable Panel version of Adult Social History checkbox in the Social History section of the Practice Configuration practice template to set the template as default. Document the Patient's Social History on Tobacco Usage

You can document a patient's social history on tobacco usage in the Social History - Tobacco template. Before you begin You must clear the Enable Panel version of Adult Social History checkbox on the Practice Configuration template to open the Social History – Tobacco template. Open NextGen® Enterprise EHR. Open the Histories template, expand the Social panel, select the Tobacco/Vaping Use link from the left pane, and then select Add/Edit.The Social History - Tobacco template opens. Note: Depending on your specialty's configurations, you can also open the Social History – Tobacco template from the navigation bar. Document the necessary details in the following panels: Tobacco Use Historical Use Efforts To Quit Tobacco Vaping Use Passive Exposure Enter any additional comments, and select OK. Add Environmental Information that Affect the Patient You can add residence, animal and miscellaneous details about the environmental factors that affect the patient on the Environmental template. M
