Document Catheterization and Intravesical Procedures

You can document the details of a catheterization or intravesical procedure.

1. On the SOAP template, select the Procedures link.
   The Procedures template opens.
2. In the Genitourinary section, select Catheterization/Intravesical Treatment.
   The Procedure - Catheterization/intravesical template opens.

   

3. In the Indication section, do the following:
   1. Select Description, and select a diagnosis.
      The code is automatically filled.
   2. Select Status, and select the diagnosis status from the list.
   3. Select Patient preppred and draped in the usual sterile fashion.
4. To enter a saved default set, select Quick Load and Save.
5. In the Pre-Procedure Care section, select the appropriate checkboxes after explaining the procedure details to the patient.
   Note: To add more information about the consent, select the Consent link.
6. In the Catheterization section, do the following:
   1. Select the catheter type, and then add comments.
   2. Enter catheter specifications and residual urine details.
   3. Enter the details of the medications and anesthetic.
   4. Select the details of the urine test.
7. In the Intravesical Treatment, do the following:
   1. Enter the details fo the treatment episode and installation.
   2. Enter the details of the anticarcinogenic agent.
   3. Enter the details of the medications and anesthetic.
8. In the Post Procedure section, enter the post-procedure details, patient instructions, and follow-up instructions.
9. Enter any additional information in Comments.
10. In Performed By, select the name of the provider who performed the procedure.
11. To submit the procedure charges, select Submit to Superbill.
12. Select Save & Close.
