Document the Patient's Location and Contact Information

You can document the patient's basic information such as the name, address, and contact information. Open the Demographics (BH) template. Enter information in the Patient Information, Contact Information, and Address History panels. Enter patient's contact details in the corresponding fields. Select Add. Managing Patient Demographic Information

In addition, you can use the Demographics (BH) template to enroll patients in programs, document COT/treatment adherence, support roles and resources, and the patient's employment details. You can select the Special Assistance checkbox if the patient needs any special care. Selecting the checkbox opens a picklist that you can configure in the BH Foundation Configuration template. After documenting the patient’s demographic information, you can select Generate Document to generate a Demographics document. Alternatively, you can select Save & Close. Update the Pregnancy History Panel

Open the Pregnancy History panel. Highlight an entry in the Pregnancy History grid. Select Add/Update. The Pregnancy Outcomes template opens. Highlight an entry in the Pregnancy history grid. Document Details for the Release of Information to the Patient

Open NextGen® Enterprise EHR, and then in Patient History, select Open Templates. The Select Template window opens. On the Medical Records tab, select All. Select Psychiatric Advance Directive (BH), and then select OK. Select the program, specialty, and visit type. Note: Only programs that the patient is actively enrolled in appear in the list. You can select <No Program> to create an encounter without a program, and then add the program later. In the Release Info panel, document the authorization details, reason for the disclosure, and the contacts involved in the release. In the Documents panel, document the details of the nature of information to be released. Document whether the patient received any privacy notice. Select Generate Document. Record Patient Demographic Data

Access the Patient Demographics template and scroll to the Patient Information panel. Patient Information Enter the following information, if not already entered: Patient Name (first, middle & last) Ethnicity Marital Status Race Country of Birth State of Birth Note: If required information is not known, enter Unknown. Reviewing Patient's History

You can view the history of the patient's medical, surgical, interim, family, and social details in the History panel. The history details in the grid are updated automatically when you add any detail in the Histories template.
