Add Employment in Demographics

Select Add. The Employment/Occupation window opens. Enter the employment details. To save and enter additional employment, select Save and then select Clear for Add. Access General Information for Patients

Open the Demographics (BH) template and expand the General panel. Enter a value in Other ID. To view past and future scheduled appointments, select View Appointment. Optional: To create account numbers for different facilities, select Acct Numbers. Optional: To print a document on the patient's face sheet, select Print Facesheet. Optional: If the patient is deceased, select from Category and Diagnosis for the cause of death. Note: You can configure the Category and the Diagnosis fields on the Dx Groups Reference Systems template. You can view of all releases that have been completed for this patient in the Release of Info History panel. Information in this grid is updated from the Release of Info (BH) template. Managing Patient Demographic Information

In addition, you can use the Demographics (BH) template to enroll patients in programs, document COT/treatment adherence, support roles and resources, and the patient's employment details. You can select the Special Assistance checkbox if the patient needs any special care. Selecting the checkbox opens a picklist that you can configure in the BH Foundation Configuration template. After documenting the patient’s demographic information, you can select Generate Document to generate a Demographics document. Alternatively, you can select Save & Close. Update the Location Contact Information

You can update the contact information for any location of your practice. On the Document Config system template, expand the Document Contact Information panel. From the left grid, select the desired practice. The locations for that practice appear in the right grid. Select the desired location. The contact information for the selected practice and location appear in the corresponding fields to the right of the grid. Update the information, as required. Click Update. Document Emergency Contacts and Patient Relationship Details

You can document emergency contact details that would play a role in the participant's foster care program. The data you enter into the Emergency Contacts/Patient Relationship section is also displayed in the Patient Information window. Open the CW Demographics (BH) panel. Expand the External Relationship Information panel. In the Emergency Contacts / Patient Relationship section, select Add. The Modify Patient Information window opens. Select Relations/Role. Modify Patient Information Window From the Relationship list, select New. The Relationship Lookup window opens. Do one of the following: To link the relationship to someone already in the system, search for the person, select it, and then select Open. To add a person to the system, enter the person's details, and then select New. The relationship record is added to the Emergency Contact / Patient Relationship grid.
