Document the Patient's Location and Contact Information

You can document the patient's basic information such as the name, address, and contact information.
  1. Open the Demographics (BH) template.
  2. Enter information in the Patient Information, Contact Information, and Address History panels.
  3. Enter patient's contact details in the corresponding fields.
  4. Select Add.
  5. Expand the Places panel, and enter location details.
  6. Select Add.
    The Location information appears in the grid.
