Components of the Demographics Tab

Icon Description Veteran Icon Appears when you select the Veteran check box for the patient on the Demographics tab of the Add/Modify Patient Information window. Smoker Icon Appears when you select the Smoker check box for the patient on the on the Demographics tab of the Add/Modify Patient Information window. Print Statement Icon Appears when you select the Print Statements check box for the patient on the Demographics tab of the Add/Modify Patient Information window. Not Enrolled in PxP Portal Pending in PxP Portal Completed in PxP Portal Deactivated in PxP Portal Account Deleted in PxP Portal Account Declined in PxP portal Displays the patient's current NextGen® PxP Portal enrollment status. Enrollment Status Indicators of Patients

NextGen® PxP Portal has the following enrollment status indicators. Status Icon Tool Tip Description Not enrolled Not Enrolled The patient is not enrolled in NextGen® PxP Portal. Not enrolled Aged Out The patient has aged out and the enrollment of guardian has been revoked. Pending Pending The practice has initiated the patient's enrollment, but the patient has not completed the enrollment. Completed Completed; Last Login Date MM/DD/YYYYNote: The last logon information appears only when the icon is available for selecting. The patient has completed the enrollment. Deactivated Deactivated The practice has deactivated the NextGen® PxP Portal account of the patient. If the account is activated again, the enrollment status changes to Completed. Deleted Account Deleted The practice has deleted the NextGen® PxP Portal account of the patient. When a patient's account is deleted, the practice can send the invitation to enroll the patient in NextGen® PxP Portal again. Declined Declined The pati

Patient Chart Lookup on the patient chart opens the Patient Lookup window. 	Indicates that you can print a statement from the patient chart. Note: Appears in patient chart when you select the Print Statements checkbox on Demographics in the Add/Modify Patient Information window.
	Indicates that the patient is not enrolled in NextGen® PxP Portal.
	Indicates that the patient NextGen® PxP Portal enrollment status is pending.
	Indicates that the patient is enrolled in NextGen® PxP Portal.
	Indicates that the patient NextGen® PxP Portal account is deactivated.
	Indicates that the patient NextGen® PxP Portal account is deleted.
