Icon	Description
	Appears when you select the Veteran check box for the patient on the Demographics tab of the Add/Modify Patient Information window.
	Appears when you select the Smoker check box for the patient on the on the Demographics tab of the Add/Modify Patient Information window.
	Appears when you select the Print Statements check box for the patient on the Demographics tab of the Add/Modify Patient Information window.
	Displays the patient's current NextGen® PxP Portal enrollment status. Patient Demographic Icons

The following table lists icons that you can view on Patient Information tab in Patient Chart. Icon Description Veteran Icon The patient is a veteran. Note: Appears in patient chart when you select the Veteran checkbox on Demographics in the Add/Modify Patient Information window. Smoker Icon The patient is a smoker. Note: Appears in patient chart when you select the Smoker checkbox on Demographics in the Add/Modify Patient Information window. Print Statement Icon Indicates that you can print a statement from the patient chart. Note: Appears in patient chart when you select the Print Statements checkbox on Demographics in the Add/Modify Patient Information window. Not Enrolled in PxP Portal Indicates that the patient is not enrolled in NextGen® PxP Portal. Pending in PxP Portal Indicates that the patient NextGen® PxP Portal enrollment status is pending. Enrollment Status Indicators of Patients

NextGen® PxP Portal has the following enrollment status indicators. Status Icon Tool Tip Description Not enrolled Not Enrolled The patient is not enrolled in NextGen® PxP Portal. Not enrolled Aged Out The patient has aged out and the enrollment of guardian has been revoked. Pending Pending The practice has initiated the patient's enrollment, but the patient has not completed the enrollment. Completed Completed; Last Login Date MM/DD/YYYY Note: The last logon information appears only when the icon is available for selecting. The patient has completed the enrollment. Deactivated Deactivated The practice has deactivated the NextGen® PxP Portal account of the patient. If the account is activated again, the enrollment status changes to Completed.
