Enter Present on Admission Indicators for UB Claims

Present on Admission Indicators (POA) are used on institutional claims to designate whether a patient's diagnosis was present at the time of inpatient admission. The claim uses the indicators entered in the Present on Admission field on the Diagnosis Selection window for each code. The POA indicator follows the diagnosis code in the 2300 loop / HI01-9 segment on electronic 837I claims, and FL 67 on paper UB04 claims. Note: The Present on Admission field is available on the Diagnosis Selection window only if UB claims have been enabled for the practice by populating the Type of Facility and Bill Classification fields in Claims preferences under the Practice Preferences window. In addition, an Institutional Billing Library must be selected for the practice in Libraries preferences under the Practice Preferences window. Access the Patient Chart for the desired patient. Add Encounter-Level Diagnosis Codes

Access the Patient Chart for the desired patient. Select the Encounters tab, and then the General sub-tab. In the Encounters list on the left, select the desired encounter. In the ICD-CM section on the right, select the Open Record button. The Diagnoses Selection window appears. Note: Existing encounter-level diagnosis codes are listed in the Selected ICD-CM Codes section on the right. Select one of the following buttons: Diagnostic to search for ICD-CM diagnostic codes. This button is selected by default. Procedural to search for ICD-PCS procedural codes. In the pane on the left, select one of the following column headings to select a search method: Code to search by diagnosis code. This column is selected by default. Description to search by diagnosis description. A flashlight icon displays in the selected column heading. In the ICD-CM Codes field, enter all or part of the desired code or description. Encounters in NextGen® Enterprise PM

An encounter is an interaction between a patient and a provider in the practice on a specific date and time. In NextGen® Enterprise PM, office encounters are typically created from the appointment book when each patient checks in for their appointment. Facility encounters that are not related to an appointment, such as hospital and nursing home visits, can be created from the patient's chart. Clinical information related to a specific encounter is documented by the provider in NextGen® Enterprise EHR. Financial information, such as charges, billing and transactions, is entered on that same encounter by users in NextGen® Enterprise PM. The Encounters tab on Patient Chart lists all encounters for a patient, and the Encounters Detail tab on Account Profile lists all encounters for a guarantor. View Diagnosis Mapping

When a diagnosis that was selected in Charge Entry multiplies into more than one code for a payer based on the alternate diagnosis mappings in the diagnosis code library, you can view the diagnosis code mapping on the General tab under Encounters in the patient chart. Open NextGen® Enterprise PM. On the main toolbar, select Chart or Chart Lookup.. The Patient Lookup window opens. In the Search Criteria section, enter the patient's information, and then select Find. Patient records that match the search criteria appears in the Matching Records section. Select the patient, and then select Open. The Patient Chart window opens. Select the Encounters tab. The encounters associated with the patient are displayed in the Encounters section. Select the encounter from the Encounters section. The details of the selected encounter appear. Select the COB Alt Diag under the Providers section. Reorder Diagnosis Codes on a Charge

You can reorder or delete charge-level and encounter-level diagnosis codes from the Charge Posting window. The Charge Posting window displays up to 12 diagnosis codes. Codes 1-4 are charge-level codes from the Charge Posting window. Additional codes 5-12 are encounter-level codes from the ICD-CM section of the General sub-tab on the Patient Chart > Encounters tab. Open NextGen® Enterprise PM. Select Charges. The Charge Posting window opens. Select the desired patient and encounter. From the charge ledger at the bottom of the window, right-click the desired charge, and then select Open. In the Charge Posting window, select the Reorder Dx link. Note: The link is available only if the selected charge has at least one diagnosis code. Add or Remove Diagnosis Codes for an Encounter

You can add or remove diagnosis codes from an existing encounter using the Payment Entry window. You cannot remove diagnosis codes if they are associated with a charge. All alternate diagnoses, such as COB1 Alt Diag, COB2 Alt Diag, and COB3 Alt Diag, appear in the Selected ICD-CM Codes list. Open NextGen® Enterprise PM. On the main toolbar, select Posting . The Batch Posting window opens. Double-click the batch header record. The Payment Entry window opens. Type the encounter number in Enc/Clm#, and press Enter. The account guarantor, patient, medical record number, and the billed account displays in the Payment Entry window. Select the Encounter Diagnosis . The Diagnosis Selection window opens.
