Add Diagnosis Codes for Encounters

Open NextGen® Enterprise PM. On the main toolbar, select Posting . The Batch Posting window opens. Double-click the batch header record. The Payment Entry window opens. In Enc/Clm#, enter the encounter number, and then select Enter. The account guarantor, patient, medical record number, and the billed account details appear in the Payment Entry window. Select Encounter Diagnosis . The Diagnosis Selection window opens. Select Search Method, and then select Code or Description. appears on the selected option. To search for a diagnosis, do one of the following: In ICD-CM Codes, enter the code number or a partial description. A list of matching diagnosis codes appear in Code and Description. To search for a phrase or number in the code value, select Search for keyphrase anywhere in the diagnosis and enter a number or partial description in ICD-CM Codes. Add or Remove Diagnosis Codes for an Encounter

You can add or remove diagnosis codes from an existing encounter using the Payment Entry window. You cannot remove diagnosis codes if they are associated with a charge. All alternate diagnoses, such as COB1 Alt Diag, COB2 Alt Diag, and COB3 Alt Diag, appear in the Selected ICD-CM Codes list. Open NextGen® Enterprise PM. On the main toolbar, select Posting . The Batch Posting window opens. Double-click the batch header record. The Payment Entry window opens. Type the encounter number in Enc/Clm#, and press Enter. View Diagnosis Mapping

When a diagnosis that was selected in Charge Entry multiplies into more than one code for a payer based on the alternate diagnosis mappings in the diagnosis code library, you can view the diagnosis code mapping on the General tab under Encounters in the patient chart. Open NextGen® Enterprise PM. On the main toolbar, select Chart or Chart Lookup.. The Patient Lookup window opens. In the Search Criteria section, enter the patient's information, and then select Find. Patient records that match the search criteria appears in the Matching Records section. Select the patient, and then select Open. The Patient Chart window opens. Select the Encounters tab. The encounters associated with the patient are displayed in the Encounters section. Select the encounter from the Encounters section. The details of the selected encounter appear. Select the COB Alt Diag under the Providers section. Reorder Diagnosis Codes on a Charge

You can reorder or delete charge-level and encounter-level diagnosis codes from the Charge Posting window. The Charge Posting window displays up to 12 diagnosis codes. Codes 1-4 are charge-level codes from the Charge Posting window. Additional codes 5-12 are encounter-level codes from the ICD-CM section of the General sub-tab on the Patient Chart > Encounters tab. Open NextGen® Enterprise PM. Select Charges. The Charge Posting window opens. Select the desired patient and encounter. From the charge ledger at the bottom of the window, right-click the desired charge, and then select Open. In the Charge Posting window, select the Reorder Dx link. Note: The link is available only if the selected charge has at least one diagnosis code. Remove Encounter Diagnoses from an Encounter to be Deleted

Access the Patient Chart. Select the Encounters tab, and then the General sub-tab. In the ICD-CM section, select the Open Record button. The Diagnosis Selection window appears. From the Selected ICD-CM Codes section, select a diagnosis code to be deleted. Select the blue left arrow button to remove the code from the encounter. A prompt appears asking to confirm the removal of this diagnosis. To remove the code, select OK. One of the following occurs: If the diagnosis code is not associated with charges, it is removed from the encounter. If the diagnosis code is associated with a charge, then a prompt appears stating that this diagnosis code is used on a charge and that it cannot be removed. For more information, see Remove Charges from an Encounter. Repeat steps 4-6 for additional diagnosis codes to be deleted. To save changes and close the window, select OK.
