Enter Present on Admission Indicators for UB Claims

Present on Admission Indicators (POA) are used on institutional claims to designate whether a patient's diagnosis was present at the time of inpatient admission. The claim uses the indicators entered in the Present on Admission field on the Diagnosis Selection window for each code. The POA indicator follows the diagnosis code in the 2300 loop / HI01-9 segment on electronic 837I claims, and FL 67 on paper UB04 claims. Note: The Present on Admission field is available on the Diagnosis Selection window only if UB claims have been enabled for the practice by populating the Type of Facility and Bill Classification fields in Claims preferences under the Practice Preferences window. In addition, an Institutional Billing Library must be selected for the practice in Libraries preferences under the Practice Preferences window. Access the Patient Chart for the desired patient. Add or Remove Diagnosis Codes

You can add or remove the diagnosis codes in the ICD-CM section on the Estimate Patient Cost window. When you add or remove diagnosis codes for existing estimates and submit the cost estimation, a cost estimate with a new cost estimation ID is created. Open the Estimate Patient Cost window for a person, patient, encounter, or appointment. In the ICD-CM section, select open record . The Diagnosis Selection window opens. In ICD-CM Codes, do one of the following to search for the desired diagnosis code. To search by code, select Code, and enter all or part of the code. To search by description, select Description, and enter all or part of the description of the code. Note: Select the Search for keyphrase anywhere in the diagnosis check box to list the codes that have the entered search criteria anywhere in the code or description. Remove Encounter Diagnoses from an Encounter to be Deleted

Access the Patient Chart. Select the Encounters tab, and then the General sub-tab. In the ICD-CM section, select the Open Record button. The Diagnosis Selection window appears. From the Selected ICD-CM Codes section, select a diagnosis code to be deleted. Select the blue left arrow button to remove the code from the encounter. A prompt appears asking to confirm the removal of this diagnosis. To remove the code, select OK. One of the following occurs: If the diagnosis code is not associated with charges, it is removed from the encounter. If the diagnosis code is associated with a charge, then a prompt appears stating that this diagnosis code is used on a charge and that it cannot be removed. Repeat steps 4-6 for additional diagnosis codes to be deleted. To save changes and close the window, select OK. Parent topic: Delete Encounters

Search All Diagnosis

Enter your search criterion in the text box at the top of the diagnosis search window. You can enter all or part of the clinical description, ICD code, or billing description. You can search for both ICD-9 and ICD-10 codes. You can enter a higher-level ICD code to display all of the associated lower-level ICD codes. Note: The search criterion may be populated from the trigger. You can change it, if necessary. If you search for ICD code 250.00, the search results include codes such as 250.01 or 250.02. Select All Diagnoses (default) in the left pane. Select Search, or select the Enter key. The search results grid displays the results that match your search criterion. Any results that match a diagnosis already on the patient’s diagnosis list display at the top followed by any results that match the provider’s most frequently assigned list. Add or Remove Diagnosis Codes for an Encounter

You can add or remove diagnosis codes from an existing encounter using the Payment Entry window. You cannot remove diagnosis codes if they are associated with a charge. All alternate diagnoses, such as COB1 Alt Diag, COB2 Alt Diag, and COB3 Alt Diag, appear in the Selected ICD-CM Codes list. Open NextGen® Enterprise PM. On the main toolbar, select Posting . The Batch Posting window opens. Double-click the batch header record. The Payment Entry window opens. Type the encounter number in Enc/Clm#, and press Enter. The account guarantor, patient, medical record number, and the billed account displays in the Payment Entry window. Select the Encounter Diagnosis . The Diagnosis Selection window opens. Reorder Diagnosis Codes on a Charge

You can reorder or delete charge-level and encounter-level diagnosis codes from the Charge Posting window. The Charge Posting window displays up to 12 diagnosis codes. Codes 1-4 are charge-level codes from the Charge Posting window. Additional codes 5-12 are encounter-level codes from the ICD-CM section of the General sub-tab on the Patient Chart > Encounters tab. Open NextGen® Enterprise PM. Select Charges. The Charge Posting window opens. Select the desired patient and encounter. From the charge ledger at the bottom of the window, right-click the desired charge, and then select Open. In the Charge Posting window, select the Reorder Dx link. Note: The link is available only if the selected charge has at least one diagnosis code.
