Add Encounter-Level Diagnosis Codes

Access the Patient Chart for the desired patient. Select the Encounters tab, and then the General sub-tab. In the Encounters list on the left, select the desired encounter. In the ICD-CM section on the right, select the Open Record button. The Diagnoses Selection window appears. Note: Existing encounter-level diagnosis codes are listed in the Selected ICD-CM Codes section on the right. Select one of the following buttons: Diagnostic to search for ICD-CM diagnostic codes. This button is selected by default. Procedural to search for ICD-PCS procedural codes. In the pane on the left, select one of the following column headings to select a search method: Code to search by diagnosis code. This column is selected by default. Description to search by diagnosis description. A flashlight icon displays in the selected column heading. In the ICD-CM Codes field, enter all or part of the desired code or description. Codes that match the entered search criteria are displayed. Note: Select the Search

Add or Remove Diagnosis Codes

You can add or remove the diagnosis codes in the ICD-CM section on the Estimate Patient Cost window. When you add or remove diagnosis codes for existing estimates and submit the cost estimation, a cost estimate with a new cost estimation ID is created. Open the Estimate Patient Cost window for a person, patient, encounter, or appointment. In the ICD-CM section, select open record . The Diagnosis Selection window opens. In ICD-CM Codes, do one of the following to search for the desired diagnosis code. To search by code, select Code, and enter all or part of the code. To search by description, select Description, and enter all or part of the description of the code. Note: Select the Search for keyphrase anywhere in the diagnosis check box to list the codes that have the entered search criteria anywhere in the code or description. Remove Encounter Diagnoses from an Encounter to be Deleted

Access the Patient Chart. Select the Encounters tab, and then the General sub-tab. In the ICD-CM section, select the Open Record button. The Diagnosis Selection window appears. From the Selected ICD-CM Codes section, select a diagnosis code to be deleted. Select the blue left arrow button to remove the code from the encounter. A prompt appears asking to confirm the removal of this diagnosis. To remove the code, select OK. One of the following occurs: If the diagnosis code is not associated with charges, it is removed from the encounter. If the diagnosis code is associated with a charge, then a prompt appears stating that this diagnosis code is used on a charge and that it cannot be removed. Repeat steps 4-6 for additional diagnosis codes to be deleted. To save changes and close the window, select OK. Parent topic: Delete Encounters

Add Diagnosis Codes for Encounters

Open NextGen® Enterprise PM. On the main toolbar, select Posting . The Batch Posting window opens. Double-click the batch header record. The Payment Entry window opens. In Enc/Clm#, enter the encounter number, and then select Enter. The account guarantor, patient, medical record number, and the billed account details appear in the Payment Entry window. Select Encounter Diagnosis . The Diagnosis Selection window opens. Select Search Method, and then select Code or Description. appears on the selected option. To search for a diagnosis, do one of the following: In ICD-CM Codes, enter the code number or a partial description. A list of matching diagnosis codes appear in Code and Description. To search for a phrase or number in the code value, select Search for keyphrase anywhere in the diagnosis and enter a number or partial description in ICD-CM Codes. Search All Diagnosis

Enter your search criterion in the text box at the top of the diagnosis search window. You can enter all or part of the clinical description, ICD code, or billing description. You can search for both ICD-9 and ICD-10 codes. You can enter a higher-level ICD code to display all of the associated lower-level ICD codes. Note: The search criterion may be populated from the trigger. You can change it, if necessary. If you search for ICD code 250.00, the search results include codes such as 250.01 or 250.02. Select All Diagnoses (default) in the left pane. Select Search, or select the Enter key. The search results grid displays the results that match your search criterion. Any results that match a diagnosis already on the patient’s diagnosis list display at the top followed by any results that match the provider’s most frequently assigned list.
