Enter Present on Admission Indicators for UB Claims

Present on Admission Indicators (POA) are used on institutional claims to designate whether a patient's diagnosis was present at the time of inpatient admission. The claim uses the indicators entered in the Present on Admission field on the Diagnosis Selection window for each code.

The POA indicator follows the diagnosis code in the 2300 loop / HI01-9 segment on electronic 837I claims, and FL 67 on paper UB04 claims.

  1. Access the Patient Chart for the desired patient.
  2. Select the Encounters tab, and then the General sub-tab.
  3. In the Encounters list on the left, select the desired encounter.
  4. In the ICD-CM section on the right, select the Open Record button.
    The Diagnoses Selection window appears.
    Existing encounter-level diagnosis codes are listed in the Selected ICD-CM Codes section on the right.

  5. Add encounter-level diagnosis codes, if needed.
  6. Select an admitting diagnosis for UB claims, if needed.
  7. To enter POA indicators, select one of the following in the Present on Admission column for the desired codes:
    • <none>: Leave blank.
    • Exempt from Reporting (1): Unreported or not used. Exempt from POA reporting. If selected, the 1 indicator prints on paper UB04 claims, but is blank on electronic 837I claims.
    • No (N): Diagnosis was not present at time of inpatient admission.
    • Not Applicable (W): Clinically undetermined. Provider unable to clinically determine whether the condition was present at the time of inpatient admission.
    • Unknown (U): Documentation insufficient to determine if the condition was present at the time of inpatient admission.
    • Yes (Y): Diagnosis was present at time of inpatient admission.

  8. To save changes and close the window, select OK.
