Select Diagnosis Data

  1. Select Diagnosis from the Report Filter Settings List.

  2. Select to display the Diagnosis Search window.
    Diagnosis Search

    • Select the pertinent Library.
    • To further narrow your search, select the Category.
    • Select whether you wish to search by ICD or Dx Code.
    • Enter a Diagnosis search criterion.
    • Select Search.
    • Select the desired Diagnoses.
    • Select OK.

      The selected diagnoses display in the Diagnosis Search pane.

  3. Select whether to Include or Exclude the displayed diagnoses.
  4. Select whether to display patients who have had problems within specific date criteria, for example:
    • Patients who have never had the problem before or after a specific date or between a specific date range.
    • Patients who have ever had a problem before or after a specific date.
    • Patients who have not had this problem since a specific date.
    • To select a date range, check Who Have Ever Had and select Between from the list.
      The Date Range window opens.

    • Select the dates.
    • Select OK.
  5. To select patients who have had problems before or after a specific date, do the following:
    • Check Who Have Ever Had and select Before or After from the list.
    • Select the Year/Month/Day. For example, 1 year and 3 months after today's date.
  6. Select the Number of Problems that have been added for a patient for the specified date range, if necessary.
  7. From the Diagnosis Filter tab, you can do the following:
    • Select whether to display All, Active or Inactive diagnosis.
    • Select Checked from the No Known Diagnosis list to display existing No Known Diagnosis information for a patient.
    • If you do not wish to display existing No Known Diagnosis data, select Not Checked from the No Known Diagnosis list.
    • Setup any desired filters (Equals, Not Equals, Greater Than, Less Than, Like) for Description, Date Onset, Date Diagnosed, Date Resolved, Note, Status and Severity.
    • Select whether the condition is Chronic or not (Y or N).
    • Select Date Time, that is, the date the record was created, for example, when a diagnosis was assigned.

    Select to display Status or Severity diagnosis search window to search for the desired filter criteria, if necessary.
  8. To add additional diagnosis data, return to the Diagnosis Search tab, select Add.

  9. Select whether the additional filter is either AND / OR.
  10. Repeat Steps 2-7.
  11. Select Columns from the Settings List and select the appropriate columns.
  12. Select Sorting or other report display options from the Report Filter, if necessary.
  13. To view the report data in alternate shading, select Alternate Shading.
  14. Select Save
  15. To generate the report, select OK.
