Medication Reasons

NextGen® Enterprise EHR uses the Medication Reasons master file as a repository for the reasons that a medication is being prescribed in the Medications Module. This reason becomes part of the Sig for the prescribed medication. File Maintenance includes a default list of reasons, but you can add custom reasons. To add a medication reason: From the Master Files > System, select Medication Reasons. The Medications Reasons List window opens. From the list, do one of the following: To add a new item, select Open Menu , and then select New. To modify an item, select the item in the list, select Open Menu , and then select Open. In the Add/Modify Medication Reasons Information window, enter the name of the reason in the Medication Reason Description field and, if necessary, enter a Note. For the reasons that were installed by default, the Note field displays the text System Generated. Add an Allergy Reaction and Severity

You can document an allergy reaction and severity for the allergy. Reaction-severity details appear wherever allergies display. Although allergy reactions and severities are SNOMED® codified, you can create user-defined reactions but not user-defined severities. Contact your system administrator for more information. From the Medication Allergies module, in the Reactions section, select Add. The Add Reaction (Severity) window opens. Reactions marked as common list first. Begin typing the name of a reaction or scroll through the list of reactions, and then select a reaction. Unknown is located at the top of the Common Reactions list. Specify a severity of the reaction. Select Add. The allergy reaction and severity appear in the Reactions section. You can add up to five allergy reaction and severity. Document Patient Allergies

You can document the allergy details of a patient. Open NextGen® Enterprise EHR. On the Intake template, in the Allergies panel, select Add. The Adult Allergies template opens. Adult Allergies template Select the applicable allergen check boxes, and then select the associated reaction from the resulting list. Note: If the patient's allergy is not available on the Allergy Reactions template, you can exit the Allergy Reactions template. You can select Save & Close to return to the Allergies panel. With the allergen selected, to open the Allergies module and document the patient's allergies, you can select Update. Select Save & Close. To enter comments about the allergen, select the Comment link and enter the details on the Allergy Comments window. CAUTION: You must not enter patient allergies manually on the Allergy Comments window. You must enter all allergies on the Allergies module. Allergies - User Defined

The Allergies master file contains a list of user-defined allergies. From the Master Files - System list, select Allergies. The Allergies - User Defined List window opens. Do one of the following: To add a new item, select open menu , and then select New. To modify an existing item, select the item in the list, select open menu , and then select Open. You can also double-click to open the item. The Add/Modify Allergy Information window opens. Enter the abbreviation for an allergy in the Allergy Key field. This field allows for a maximum of 8 characters. Enter the full description of the allergy in the Allergy Description field. Enter any additional information for the allergy in the Note field. If you want to hide the item in the list, select the Hide check box. Select Next to add another item or select OK to return to the Allergies - User Defined List window. Document New Allergies for a Patient

You can document allergy details of a patient. Before you begin If the Joint Commission standards are enabled on the Practice Configuration template, document allergy information after entering whether the patient is new or established along with the Visit Type and Historian. After documenting allergies, continue entering other details. Open NextGen® Enterprise EHR. On the Intake template, in the Allergies panel, select Add. The age-specific allergies template opens. Select the applicable check boxes, and then select the associated reaction from the corresponding list. If the patient's allergy information is not available on the template, select Manage Allergies and document the allergy details. If you want to document the allergy severity and additional details, select Manage Allergies. A message appears indicating to confirm if you want to add the selected allergy to the Medication Allergies module. Add a Medication Allergy to a Patient Chart

Open NextGen® Enterprise EHR. Open the Medication Allergies module for the selected patient. If the patient has any existing allergies, select Clear. The values in all the fields are cleared. Note: Clearing the fields prevents you from editing an existing allergy. To search and filter an allergy, do the following: Select the search icon next to Allergy. The NextGen - Allergy Selection window opens. Allergy Selection Select one of the following filters from the Filter list: Contains Searches for allergies that contain with the search criteria. Ends With Searches for allergies that end with the search criteria. Ingredient Searches from the list of drug ingredients, such as Iodine or Bentonite. Medication Name searches from the list of medication allergies, such as cefazolin with codeine. Allergen Group Searches from the list of allergens, such as lithium derivatives or ester type anesthetics.
