Add a New Service Item

On the Service Item Library Maintenance window, select open menu button , then select New. Enter a CPT code or select on the to search for the CPT code. If you search for the code, the CPT4 Code Search window opens. Under Search Method, select either Code or Description to determine how you want the search results to list, then enter the search data either by code or description. Select the appropriate code, and then select OK. The service item number and default description automatically populate. The default description for the SIM is the same as the CPT4 code description. You can change the description if necessary. For example, if the description Office/outpatient visit, new, problem focused is not meaningful to the staff, you could change it to simply Office Visit New Patient. Enter the information on the General tab, including effective date range and price. Required fields display with red labels. Patient Alert Message

A patient alert message displays whenever you access an encounter during the after care period and enter a charge for the service item that has an after care period set. The message displays to help prevent the accidental posting of additional charges during the after care period. In addition, if a service item is entered multiple times on the Charge Entry window in NextGen® Enterprise PM, the after care alert only displays once. When a rendering physician is attached to a service item with an after care period, then the rendering physician's name displays in the after care alert. Note: If a service item is entered multiple times on the Charge Entry window, but each entry has a different service date, then an after care alert displays for each of the entries. Add SIM Items to a SIM Library

After you have created a SIM library, you need to add service items and pricing information to the library. Select File Maintenance > Libraries > Service Items. From the Service Items List window, select the library to update. The Service Item Library Maintenance window opens. Select the open menu and select New. The three service item fields turn red. In the CPT4 Code field, enter a CPT4® code from your list of service items and then select Enter. The details for that CPT4® Code appears. In the General tab, do the following: In Eff Date, enter the effective date. In Exp. Date, enter the expiration date. In Non-Facility Price, enter the non-facility price in dollars. In Facility Price, enter the facility price in dollars. If you have more items to add, select Enter and repeat steps 4 to 6 for all service items you wish to add. Create SIM Groups in a Service Item Library

Orders module charge posting also looks at SIM Groups which are set up in File Maintenance in the Service Item Libraries. You must first define a Service Item library, and add service items to the library before you can create a SIM group. Select File Maintenance > Libraries > Service Items. Right-click the Service Item library for which you want to create a SIM Group. The Service Items List window opens. Select SIM Groups from the list. Select the open menu button, and then select New. The SIM Group Maintenance window opens. Define the SIM Group ID and Description. SIM Group. This is a numeric or alphabetic ID that you use to identify this SIM Group. Description. The description you choose for this SIM Group. Service Item #. This is the list of service items contained in this group. Select in the Service Item #. Service Item Library Maintenance Field Descriptions

The following table describe the fields for setting up a Service Item Library. Field Description Search You can search for any specific SIM. If there are less than 50 SIM items defined in the SIM Library, all active SIM items automatically appear in the Search list. If there are more than 50 SIM items defined in the SIM Library, the SIM codes do not appear automatically. You must manually enter the SIM code or the first few digits of the code in the Search field. To find a specific SIM, enter the SIM code. To list all existing SIMs, enter an asterisk. SIM Library Name SIM Library name is a required field. Service Item For a new SIM, you can enter meaningful service item numbers and descriptions to correlate to the appropriate CPT4 code. This field is required. When you search for a SIM, this field displays the SIM that you search for. You can also select the open menu button, select New, and then enter a new service item Number. Hide SIM You can select this check box to hide the SIM. I

Set Up Anesthesia SIMs

Before you can use anesthesia billing, service items billed as anesthesia services must be designated as anesthesia SIMs. You must also designate the appropriate claim form type and the number of base units. In File Maintenance, select Libraries and then Service Items. The Service Items Libraries window opens. Double-click the appropriate service item library. The Service Item Library Maintenance window opens. Under Service Item #, select the open menu button, and then select New. Enter an applicable name for the new anesthesia SIM, in the Description field. In the CPT4 Code field, select or enter the correct CPT4 code. On the General tab, complete the following fields. Field Description Form Select the 1500 or UB claim form type for the selected SIM. Note: Anesthesia billing is not supported for ADA claims in NextGen® Enterprise PM.
