Practice Level Significant Events Reference List
The following table describes the Practice Level Siginificant Events.
Field | Description |

Account Accessed | Logs each time a user opens an existing person or patient account. To view the significant event record, open the Patient Account Profile and then select the Notes tab > Account Notes > History. The event is logged as Account Accessed. |
Account Form Template Printed | Tracks when an account form template is printed. |
Allergy Resolved | Tracks when a patient allergy record is resolved. |
Appointment Accessed | Logs each time a user opens an existing appointment. The event logs the appointment date and time and patient name. |
Appointment Conflict Override | Logs each time a user successfully overrides a scheduling conflict. |
Appointment Letters Printed | Tracks when appointment letters are printed. |
Appointment Waitlist Completed Successfully | Tracks when an appointment waitlist item has been completed. |
Appointment Waitlist Created | Tracks when an appointment waitlist has been created. |
Appointment Waitlist Deleted | Tracks when an appointment waitlist has been deleted. |
Balance Control Change | Tracks any time a user accesses the Balance Controlwindow and modifies the charge buckets or the rebill indicators. The event appears on the Balance Control and Encounter Insurance Selection windows as Balance Control Change - Encounter: <encounter number> from: <previous charge information> to <current charge information> |
Batch Account Collection Letters Printed | Tracks when account-level collection letters are printed. |
Batch Encounter Collection Letters Printed | Tracks when encounter-level collection letters are printed. |
Budget Letters Printed | Logs an event when a budget letter is printed, the person or patient on the budget plan, and the letter type. If a Normal budget letter is printed, the event includes the old and new due dates. |
Budget Payment Posted | Logs an event when payments are posted to the budget and the number of remaining payments. |
Budget Plan Disabled | Logs the disabled budget plan and the reason it was disabled. |
Budget Plan Established | Logs new and re-established budget plans and their terms. |
Budget Plan Terminated | Logs when a budget plan is terminated. |
Budget Print Date Reset | Tracks when the Reset button was last selected. Selecting Reset enables you to reprint a batch of budget letters/statements from a specific date. |
Budget Statement Printed | Tracks when a budget statement has been printed. |
Charge Credit Date Updated | Tracks when the credit date is updated. Credit date is determined when a line item reaches a negative balance. The event logs the previous value, the new value, and the amount of the credit. |
Charge Field Modified | Logs changes to the following Charge Posting fields for charges being applied to encounters and invoices.

Charge Outsource Status Reset | Logs when an outstanding outsourced charge has been reset and is no longer being outsourced to a collection agency. The event logs the patient name and the associated encounter and SIM for the Significant Events report. In the patient chart, the event is logged as Charge Outsource Status Rest - Encounter: <Enc #> Charge SIM: <SIM code> on the Clinical History/Notes tab > Chart Notes > History > Encounters. |
Charge Outsourced | Logs when an outstanding charge has been outsourced to a collection agency. The event logs the patient name and the associated encounter and SIM for the Significant Events report. In the patient chart, the event is logged as Charge Outsourced - Encounter: <Enc #> Charge SIM: <SIM code> on the Clinical History/Notes tab > Chart Notes > History > Encounters. |
Chart Accessed | Tracks when a user opens a patient chart in NextGen® Enterprise PM. |
Chart Accessed in EHR | Tracks when a user opens a patient chart in NextGen® Enterprise EHR. |
Chart Created | Logs when a patient chart is created. The event logs the patient name, the user who created the chart, and the creation date. |
Chart Created in EHR | Tracks when a new chart is created in NextGen® Enterprise. |
Chart Guarantor Changed | Tracks changes to the guarantor on the patient chart > Relationship/Contact/Guarantor tab and records the event in the Patient Chart on the Clinical History/Notes tab > Chart Notes > History > Chart. |
Chart UD1-8 | These eight Chart UD significant events track when changes are made to the chart user-defined fields. The names of the significant events match the client-defined names entered in Chart Practice Preferences. The event tracks the previous and current values of the fields and is logged as: Chart UD# from: <none> to: <Field Name>. |
Claim Created | Logs when a claim was created, when batch claims are printed, whether the claim was clean or dirty, the payer, and the media type. |
Claim Deleted From Encounter | Claim suppressed due to payer master indicator. |
Claim suppressed due to payer master indicator | Tracks when a claim is suppressed due to a payer master indicator. |
Copy of Archived Claim Printed | A practice-level significant event is recorded when archived claims are reprinted. This information is recorded only when the Actual Claim Form is selected on the Claim Printing window. |
Credit Card Credit Processed | Tracks when a credit card payment is credited. Tracking information includes transaction date, active card type, last four digits of the card number, and expiration date. |
Credit Card Sale Processed | Tracks when a credit card payment is completed. Tracking information includes transaction date, active card type, last four digits of the card number, and expiration date. |
Credit Card Void Processed | Tracks when a credit card payment is voided. Tracking information includes transaction date, active card type, last four digits of the card number and expiration date. |
Credit Card on File Enabled | Tracks when a credit card becomes the active card on file. Tracking information includes card type, last four digits of the card number, date the card became active, and expiration date. |
Credit Card on File Disabled | Tracks when a credit card becomes disabled or is no longer the active card on file. Tracking information includes card type, last four digits of the card number, date the card is disabled, and expiration date. |
Custom Dunning Message Changed | Tracks when a custom dunning message has been changed. |
Diagnosis Resolved | Tracks when a patient diagnosis is resolved. |
Document Deleted | Tracks the deleted document. |
Document Exported | Tracks when documents are exported. You can view the event on the ICS Sig Event report or by right-clicking an image and selecting Image Properties. |
Eligibility Verification Issued | Logs when an eligibility verification is submitted from NextGen® Enterprise PM. |
Eligibility Verification Response Received | Logs the results of an eligibility submitted using NextGen Eligibility Verification. |
Employer Insurance Change | Logs when the insurance that is associated with a patient's employer has been changed on the Employer Maintenance window. The event tracks the patient name, previous insurance, and current insurance for the Significant Events report. In the patient chart, the event is logged as Employer Insurance Change on the Clinical History/Notes tab > Chart Notes > History > Chart. |
Encounter Accessed | Logs each time a user opens an existing patient encounter. To view the significant event record, open the patient Account Profile and then select the Clinical History/Notes tab > Chart Notes > History > Encounters. The event is logged as Encounter Accessed - Encounter: <Enc #>. |
Encounter Charge Deleted | Tracks when encounter charges have been deleted. |
Encounter Collection Agency Change | Tracks when the collection agency is changed for an encounter in “bad debt” status in the Agency Bad Debt Maintenance window. |
Encounter Deleted | Logs encounters deleted from the patient chart and the associated encounter numbers. To view the significant event record, select the Patient Chart and then select Clinical History/Notes tab > Chart Notes > History > Chart. The event is logged as Encounter Deleted - <Enc #>. |
Encounter Field Modified | Creates encounter-level significant events when users make changes to the Encounter Maintenance window > Billable Date, Billable Time, Occurrence Code, State, Onset Date, and Onset Time fields and to any fields on the General tab. |
Encounter Guarantor Changed | Tracks changes to the guarantor on the patient chart > Encounters tab > General subtab and records the event in the Patient Chart on the Clinical History/Notes tab > Chart Notes > History > Encounters. |
Encounter Insurance Deleted | Tracks when a user deletes an insurance from an encounter on the Patient Chart > Encounters tab > Insurance subtab. The event appears on the Balance Control and Encounter Insurance Selection windows as Encounter Insurance Deleted: <insurance name> - Encounter: <encounter number>. |
Encounter Insurance Re-Ordered | Creates a practice-level significant event to track when an encounter insurance is reordered through Balance Control when encounter insurance is added or removed on the encounter level (Patient chart > Encounters tab > Insurance subtab > New) when an insurance is deleted on the encounter level (Patient chart > Encounters tab > Insurance subtab > Delete) . The event appears on the Balance Control and Encounter Insurance Selection windows as Encounter Insurance Re-Ordered - <encounter number>. |
Encounter Locked | Tracks when an encounter has been locked. |
Encounter Rate Adjustment Reversed | Tracks when encounter rate auto adjustments are reversed from an encounter. |
Encounter Rate SIM and Amount Removed | Tracks when an Encounter Rate SIM code is removed from an encounter |
Encounter Status Change | Records the following status changes and the processes that generates them: Billed – Claim requested, statement printed Bad Debt – Process through the menu History – All charges that have no balance In Progress to Unbilled – Process that converts an encounter status from "In Progress" to "Unbilled" Rebill – Manually processed through Balance Control The event appears on the Balance Control and Encounter Insurance Selection windows as Encounter Status Change - Encounter: <encounter number> from: <previous status> to <current status>. |
Encounter Unlocked | Tracks when an encounter has been unlocked. |
Form Template Printed | A significant event is recorded when a form is printed from any one of the following areas of the application: Patient Chart, Encounter Lookup, Appointment List, Appointment Book, Charge Posting, Payment Entry, Worklog Manager, and Account Lookup. The significant event can be tracked and viewed at the account, chart, and encounter levels. |
Insurance Not Confirmed During Autoflow | Tracks when a user does the following actions during Autoflow checkout:

Invoice Charge Deleted | Tracks when an invoice charge has been deleted. |
Invoices Printed | Tracks when an invoice has been printed. |
Label Template Printed | A significant event is recorded when a label is printed from any of the following areas of the application: Patient Chart, Encounter Lookup, Appointment List, Appointment Book, Charge Posting, Payment Entry, Worklog Manager, and Account Lookup. The significant event can be tracked and viewed at the account, chart, and encounter levels. |
Medical Necessity Created | Tracks when Medical Necessity requests are submitted. To view the significant event record, select the patient chart and then select Clinical History/Notes tab > Chart Notes > History > Encounters. Medical Necessity tracking records the date and time, creator, and a description for each Medical Necessity query. |
Medication Renewed | Tracks when a medication has been renewed for a patient. |
Medication Stop | Tracks when a medication has been stopped for a patient. |
Merge Accounts | Records the account merge when a person is merged. You can view the event in NextGen® Enterprise PM in Account Profile > Notes > History. |
Merge Charts | Records the chart merge when a person is merged. You can view the event in NextGen® Enterprise PM in Patient Chart > Clinical History/Notes > Chart Notes > History. |
Merge Encounters | Records all merged encounters that occur during a person merge. You can view the event in NextGen® Enterprise PM in Patient Chart > Clinical History/Notes > Chart Notes > History > Encounters. |
New Encounter Created | Tracks when a new encounter has been created. The event logs the encounter number, patient name, the user who created the encounter, and the creation date. |
Page Annotated | Creates a significant event when annotations to a page are saved in NextGen® Document Management. Additionally, annotations on a NextGen® Document Management image in NextGen® Enterprise are tracked when a signature is added or the image is rotated. |
Page Appended | Creates a NextGen® Document Management related significant event when users add a page to a filed document. To view the significant event record, select the patient chart and then select Clinical History/Notes tab > Chart Notes > History > Document Management. The significant event record also appears on the Image Properties window in NextGen® Document Management. |
Page Filed | Creates a NextGen® Document Management related significant event when users file document pages in NextGen® Document Management. To view the significant event record, select the patient chart and then select Clinical History/Notes tab > Chart Notes > History > Document Management. |
Page Moved | Creates a NextGen® Document Management related significant event when a user moves a page from one batch of documents to another. To view the significant event record, select the patient chart and then select Clinical History/Notes tab > Chart Notes > History > Document Management. |
Page Re-Filed | Creates a NextGen® Document Management related significant event when users re-file document pages in NextGen® Document Management. To view the significant event record, select the patient chart and then select Clinical History/Notes tab > Chart Notes > History > Document Management. |
Page Viewed | Creates a significant event when users view filed pages in NextGen® Document Management that are indexed to a patient. |
Patient Allergy Deleted | Creates a practice-level significant event to track when an allergy is deleted from a patient's allergy list. |
Patient Procedure Activity On a Locked Encounter | Tracks when a user creates, updates, or deletes a charge within NextGen® Enterprise PM or other external interface and the change automatically updates the patient procedures in NextGen® Enterprise EHR when the encounter is locked in NextGen® Enterprise EHR. The significant event tracks the encounter, service item number, and date of service as A patient procedure was inserted onto a locked encounter. Encounter: <number>, service item id: <number>, date of service: <mm/dd/yyyy>. |
Patient Status Change | Logs when a user changes a patient status on the Patient Information window > Status tab. The event logs the patient name and the previous and current statuses for the Significant Events report. In the patient chart, the event is logged as Patient Status Change from: <Previous Status> to: <Current Status> on the Clinical History/Notes tab > Chart Notes > History > Chart. |
Privacy Notice Information Change | Tracks changes to Privacy Notice information as a Significant Event. Privacy Notice tracking records the date and time, creator, and a description for each change made to Privacy Notice information. To display the significant event record, select the Patient Chart and then select Clinical History/Notes tab > Chart Notes > History > Chart. |
Real-time Adjudication Request Submitted | Tracks when real-time adjudication requests have been submitted. |
Recall Letters Printed | Tracks when recall letters have been printed. |
Recall Print Date Reset | Recall Print Date Reset significant event is recorded in each patient's chart. When a batch of recall letters is printed/exported and the Updated Dates button is selected, each patient's recall plan is updated with a 1st Mailing Date, 2nd Mailing Date or 3rd Mailing Date. At that point, the recall plans in the batch no longer qualify for a letter. |
Re-establish Budget Plan | Tracks when a budget plan has been re-established. |
Referral Request Issued | Tracks when referral request responses have been issued. |
Referral Request Response Received | Tracks when referral request responses have been received. |
Relationship Deleted | Tracks when relationships are deleted from the patient chart Relationship/Contact/Guar tab. |
Relationship Hidden | Tracks when relationships are hidden in the patient chart Relationship/Contact/Guar tab. |
Reviewed Document Signed | Creates a NextGen® Document Management related significant event. |
Started For The First Time | Tracks when a NextGen® Document Management process has been started for the first time. |
Statement Counter Reset | Creates a practice-level significant event when the statement counter is reset to zero on the Account ProfileProperties tab. The event states the old and new values. |
Statement Print Date Reset | Indicates the statement print date was reset so statements can be regenerated from a specific date. |
Statements Printed | Tracks when statements are printed in batch or on-demand and the user answers Yes to the prompt "Did the statements print correctly?" The event tracks last "next statement" date, new next statement date, demand or batch mode, and whether "Generate Statement Next Run" is on or off. |
Task Field Modified | Creates chart-level significant events when users make changes on the Add/Edit Task window > Task Info tab and the Task Approval tab. The event and the old and new settings are recorded on the Add/Edit Task window > History tab. |
Unposted Transaction Deleted | Tracks when an unposted transaction is deleted from a batch. If a credit card was used for the transaction, the significant event record includes the last four digits of the credit card and the TransFirst® transaction number. |
Unposted Transaction Modified | Tracks when any of the following fields are modified for an unposted transaction in Payment Entry:
As it tracks who changes the transactions, this significant event can help to prevent fraud. |
Un-Reviewed Document Signed | Creates a NextGen® Document Management related significant event. |
