Calculate and Submit the CPT Codes for Transition of Care Visits

You can calculate and submit the CPT codes for the transition of care visits. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Select Specialty as Family Practice and Visit Type as Transition into Care. On the Intake template, select the Medications panel. Select the Medications Reconciled checkbox. Select the Finalize tab.The Finalize template opens. Evaluation and Management Coding panel on the Finalize template Under the Today's Assessment panel, add an assessment, if an assessment has not been added. Select the Evaluation and Management Coding panel.Notice that the Transition to Care Face to Face Visit checkbox is selected by default (even if it is not a face-to-face visit). This does not appear in any other place. In Medical Decision Making, select a complexity level. You can document transitioning into care from any specialty affected by transition into Care Management. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Open the Intake template. Select a Specialty that is affected by Transitioning into Care Management and select Visit Type as Transition into Care. In the General Information panel, select the Transitioning Into Care link.The Transitioning Into Care template opens. You can document transitioning into care from the Care Management specialty. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Open the Intake template. Enter Counseling Details

You can enter the counseling details for a patient and include them in the visit note. Open NextGen® Enterprise EHR. Open a patient record. Open the Finalize template. On Evaluation and Management Coding, select the Counseling Details link.The Counseling Details template opens. In the Counseling/Educational Details section, do the following: Select the counseling type, method, and evaluation.The counselor's name and date appear. Enter the time spent in patient counseling. Enter additional counseling information. Enter the type of educational materials the patient received. To enter details about an interpreter, select the corresponding checkbox, and then enter their language, name, and relationship with the patient. Submit No Charge Claims

You can use the No Charge Visit checkbox in the Evaluation and Management Coding panel on the Finalize template to submit the no-charge SIM code when you do not want to charge a patient encounter. Before you begin The No Charge Visit checkbox is configured by default. You must configure a no-charge code defined in the SIM library using the Manage Procedure Groups and Codes template. Submit E&M Codes under 95 or 97 Coding Guidelines

You can submit a calculated or selected E&M code to the Superbill under the 95 or 97 Coding Guidelines. Before you begin You must select an assessment in the Today’s Assessments panel to submit an E&M code. Open NextGen® Enterprise EHR. For a patient record, open a new encounter. Open the Finalize template. Expand the Evaluation and Management Coding panel. Evaluation and Management Coding To use a calculated E&M code, you can select the MDM level, or enter the total visit time. Enter any additional visit codes. To meet visit code criteria for Quality Reporting, select a SNOMED® CT code. To override the calculated code, select an E&M code in the code options section at the end of the Evaluation and Management Coding panel. Note: The 99201 code cannot be selected as the code has expired on January 1, 2021.
