Select SNOMED® CT Codes for Quality Reporting

You can use SNOMED® CT® codes for Quality Reporting. If you do not bill from NextGen® Enterprise EHR but need to meet the visit-code criteria for Clinical Quality Measures (CQMs), you can use a SNOMED® CT code instead of a CPT® code. Open NextGen® Enterprise EHR. For a patient record, open a new encounter. Open the Finalize template. On Evaluation and Management Coding, select the SNOMED Type (optional) link.The SNOMED Type template opens. Select a SNOMED® CT code. Select Save & Close.The NextGen® HQM application uses the selected code at the registry reporting time. Note: The code is not uploaded to the Procedures or Problems modules. Overview of Evaluation and Management Coding

You can calculate and submit the E&M code. You can submit and calculate the Evaluation and Management (E&M) code from the Finalize template using the 95, 97, 2021 Guideline, or 2023 Coding Guidelines update based on the Practice Configuration template settings. The available E&M codes vary by specialty and visit type. If you do not want to charge a patient encounter, you can select the No Charge Visit checkbox in the Evaluation and Management Coding panel to submit a no-charge code. The No Charge Visit procedure group checkbox is configured by default. The No Charge Visit checkbox is available only if the no charge SIM code is configured to the No Charge Visit procedure group using the Manage Procedure Group and Codes template. Submit E&M Codes under 95 or 97 Coding Guidelines

You can submit a calculated or selected E&M code to the Superbill under the 95 or 97 Coding Guidelines. Before you begin You must select an assessment in the Today’s Assessments panel to submit an E&M code. Open NextGen® Enterprise EHR. For a patient record, open a new encounter. Open the Finalize template. Expand the Evaluation and Management Coding panel. Evaluation and Management Coding To use a calculated E&M code, you can select the MDM level, or enter the total visit time. Enter any additional visit codes. To meet visit code criteria for Quality Reporting, select a SNOMED® CT code. To override the calculated code, select an E&M code in the code options section at the end of the Evaluation and Management Coding panel. Note: The 99201 code cannot be selected as the code has expired on January 1, 2021. The Finalize Template Workflow

You can do the following on the Finalize template: Submit and review the plan of care, based on your role, on the Provider Sign Off panel. Document Evaluation and Management (E&M) codes on the Evaluation and Management Coding panel. Calculate the patient's medical-decision-making level and submit the E&M codes to the Superbill module. Review the patient's history and physical examination details on the E&M Level panel and identify factors in the calculation of the E&M code under the 95 or 97 Coding Guidelines. Evaluation and Management Guidelines on Finalize

You can view different coding guidelines on the Finalize template. The guidelines are available as the following panels in the template. 95 Physical Exam Details displays the calculated E&M points for each listed component. 95 Physical Exam Details on Finalize 97 Multi System Physical Exam Details displays detailed information on calculating points for multi system physical exam items to calculate a total E&M service code. 97 Multi System Physical Exam Details on Finalize 97 Single System Physical Exam Details displays detailed information on calculating points for single system physical exam items to calculate a total E&M service code. You can only use one of these guidelines at a time. You need to configure the guidelines in the E&M Coding section on the Practice Configuration template. Enter the CPT Codes for Transition of Care Visits

Open NextGen® Enterprise EHR. Select a patient and open an encounter. On the Intake template, select an affected Specialty (Family Practice/Internal Medicine, Obstetrics or Pediatrics), and select Visit Type as Transition into Care. Select the Finalize tab.The Finalize template opens. In the Evaluation and Management Coding panel, select the second Value Code field.A list of service categories opens. Evaluation and Management Coding Select Care Management.A list of visit codes and descriptions opens. Select the visit code that represents the complexity and timing of care. Select OK.After returning to the Finalize template, the CPT code fills the second Visit Code field. Important: You must complete all the fields in the Transitioning Into Care or the Cm Care Transitions template before submitting the charges to the Superbill.
