Overview of the Finalize Template Workflow

You can complete the encounter workflow using the Finalize template. The template allows you to do the following: Calculate all the elements documented in the Intake, Histories, and SOAP templates. Select the Evaluation and Management (E&M) code level for the visit. Submit the E&M coding to the Superbill module. Tobacco Cessation You can use the Tobacco Cessation template to document the patient's tobacco use and the methods used for tobacco cessation. Patient's General Information on Finalize You can view the general details related to finalizing the workflow of the patient in the General panel. Patient's Assessments on Finalize You can view, add, or edit patient assessments in the Today's Assessment panel. Evaluation and Management Level Summary You can view the summary of a patient's history, exam levels, and coding points for the Evaluation and Management (E&M) code calculation under the 95 or 97 Coding Guidelines in the E&M Level Summary panel. Finalize Template

The Finalize template allows the provider to calculate all of the elements that were documented in the Intake, Histories, and SOAP tabs, select the Evaluation and Management (E&M) code level for the visit, and submit the E&M coding to the Superbill module. Patient's General Information on Finalize You can view the general details related to finalizing the workflow of the patient in the General panel. Patient's Assessments on Finalize You can view, add, or edit patient assessments in the Today's Assessment panel. Provider Sign Off You can submit and review the plan of care based on your role in the Provider Sign Off panel. You can view the summary of a patient's history, exam levels, and coding points for the Evaluation and Management (E&M) code calculation under the 95 or 97 Coding Guidelines in the E&M Level Summary panel. You can review the history and physical exam information on E&M Level Summary to see which elements factor into the calculation of the Evaluation and Management (E&M) code under the 95 or 97 Coding guidelines. On E&M Level Summary, you can view the following: Calculated patient history level Calculated physical exam levels Total number of points assigned to elements for history details 95 physical exam details, or 97 physical exam details for multiple systems or a single system If the 2021 Coding Guidelines are in effect, you can refer to E&M Level Summary to determine if you have documented a medically appropriate history and physical exam elements; however, the history and physical exam elements are not included in the calculation of the E&M code. Cardiology Provider Workflow on the Finalize Template

The Finalize template allows you to document various patient findings. You can use the Finalize - Cardiology template to do the following. Document assessments, medical decision making, counseling details, and Evaluation and Management (EM or E&M) coding. Submit codes for billing. Open a patient support case, visit document, and referral thank-you letter. Evaluation and Management Coding You can use three types of guidelines to calculate the Evaluation and Management code for Cardiology. Generate Documents You can generate and print various patient documents such as patient support cases, visit documents, referral thank-you letters, and provider communications on the Finalize - Cardiology template. Review and Sign Off Care Plans The Provider Sign Off section enables you to send a request to the supervising physician for review and sign off to indicate that the plan of care provided to the patient is appropriate. Patient's General Information on Finalize

You can view the general details related to finalizing the workflow of the patient in the General panel. In the General panel, you can do the following: View the patient type. If other encounters exist for the patient, the Established patient option is selected, and if no other encounters exist, then the New patient option is automatically selected. You can also change the selected patient type. The calculated E&M codes are based on the patient type. View the type of guidelines (1995, 1997, or 2021) selected for Evaluation and Management (E&M) coding. Opens information on E&M guidelines for 1995 or 1997 from the Centers for Medicare and Medicaid Services (CMS) website, and for 2021 from the American Medical Association (AMA) website using the Web button. Select the physical exam type for multiple systems or a single system, if you turned on the 97 Coding Guidelines on the Practice Configuration template. In the General panel, if you select Single system, you need to select an exam type

History Details on Finalize

You can view the coding points assigned to each element of the patient's history component under the 95 or 97 Coding Guidelines in the History Details panel. The coding points are factored into the calculation of the Evaluation and Management (E&M) code under the 95 or 97 Coding Guidelines. If the 2021 Coding Guidelines are in effect, you can refer to this panel to review the history you documented; however, the history elements do not factor into the calculated E&M code. You can view the following information about history details. History of presenting illness Chronic conditions Past medical, family, and social history Review of systems Points Assigned to History Details You can view the points assigned to the history of presenting illness; chronic conditions; past medical, family, and social history; and review of systems in the History Details panel.
