Enter the CPT Codes for Transition of Care Visits

Open NextGen® Enterprise EHR. Select a patient and open an encounter. On the Intake template, select an affected Specialty (Family Practice/Internal Medicine, Obstetrics or Pediatrics), and select Visit Type as Transition into Care. Select the Finalize tab.The Finalize template opens. In the Evaluation and Management Coding panel, select the second Value Code field.A list of service categories opens. Select OK.After returning to the Finalize template, the CPT code fills the second Visit Code field. Document Transitioning into Care from Any Specialty

You can document transitioning into care from any specialty affected by transition into Care Management. Open NextGen® Enterprise EHR. Select a patient and open an encounter. Open the Intake template. Select a Specialty that is affected by Transitioning into Care Management and select Visit Type as Transition into Care. In the General Information panel, select the Transitioning Into Care link.The Transitioning Into Care template opens. Select the Initial Communication field, and select the appropriate option from the list of communication types. You can use the Transition Into Care visit type to document the medical and psychosocial problems that require moderate to high-complexity decision making related to different types of discharge. You can document the transition into care details on the Transitioning Into Care template along with other relevant details and verify if the appropriate CPT codes have been calculated and submitted to the Superbill. You can find the transitioning into care management services for the following specialties: Family Practice/Internal Medicine Pediatrics Obstetrics To bill for transitioning into care management, you can use one of the following two CPT codes: 99495 for transitional care management services with moderate medical decision complexity (face-to-face visit within 14 days of discharge). 99496 for transitional care management services with high medical decision complexity (face-to-face visit within 7 days of discharge). Submit No Charge Claims

You can use the No Charge Visit checkbox in the Evaluation and Management Coding panel on the Finalize template to submit the no-charge SIM code when you do not want to charge a patient encounter. Before you begin The No Charge Visit checkbox is configured by default. You must configure a no-charge code defined in the SIM library using the Manage Procedure Groups and Codes template.
