Adding Patient Relationships, Contact, or Guarantor Information

A relationship is any person with a connection to the patient who is using (or could use) his or her insurance to cover the patient charges. Relationships can include parent child, sibling, and non familial support role as a caregiver. A patient can have more than one relationship connected to his or her chart. For example, if a child is covered by the parents' insurance, both parents can be entered in the relationships list. A patient can have a relationship to a guarantor, insurance policy holder, emergency contact and head of the household. Information that you add to the Default Guarantor and Contact Information window automatically creates a record in the patient chart. Relationships are enterprise-wide. When a relationship is created in a different practice, that relationship appears when you create a chart for your practice. Add Notes to Encounters

Open NextGen® Enterprise PM. On the main toolbar, select Chart or Chart Lookup . The Patient Lookup window opens. In the Search Criteria section, enter the patient's information, and then select Find. Patient records that match the search criteria appear in the Matching Records section. Select the patient, and then select Open. The Patient Chart window opens. Select Encounters and then select the Notes tab under it. The Encounters and Encounter Notes sections appear. Under the Encounters section, select an encounter. Select open record and then select New. The Patient Note window opens. Patient Notes Window Enter details in Subject and Note Entry section, and then select OK. Note: You can select the Seal Note check box to seal and keep a note confidential. Attach Notes to Documents and Pages

Before you begin You must do the following in System Administrator: Configure Create Quick Note operation right. 0Select the Seal Chart Notes operation right. Open NextGen® Document Management. On the toolbar, select Search, and then search for the documents or images. The Matching Records grid displays the list of patients who are associated with images that match your search results. Double-click a patient record to open its associated images in the Search Results For: window, and then double-click the image you want to view. The NextGen® Document Management viewer displays the image. From the image viewer toolbar, select the NotesNotes icon, and then in the Note Details window, select Document Notes or Page Notes.The Quicknote: Document Notes or Quicknote: Page Notes window opens. Create Quicknotes

Open NextGen® Enterprise PM. On the main toolbar, select Chart or Chart Lookup . The Patient Lookup window opens. In the Search Criteria section, enter the patient's information, and then select Find. Patient records that match the search criteria, appear in the Matching Records section. Select the patient, and then select Open. The Patient Chart window opens. Select the Clinical History/Notes tab. The Clinical History/Notes window opens displaying Topics and Description. Under the Topics section, select the category to attach the note and then right-click in the Descriptions section and select Quicknote. The Quicknote window opens. Enter a subject for the note. Select a description for the user note from the User Note Description list. Note: User Note Descriptions are automatically filled text items that you can select when you create a note. Delete Encounter Notes

Open NextGen® Enterprise PM. On the main toolbar, select Chart or Chart Lookup . The Patient Lookup window opens. In the Search Criteria section, enter the patient's information, and then select Find. Patient records that match the search criteria appear in the Matching Records section. Select the patient, and then select Open. The Patient Chart window opens. Select Encounters and then select the Notes tab. The Encounters and the Encounter Notes sections appear. Under the Encounter Notes section, right-click the note and then select Delete. A message appears asking you for a confirmation to delete the note. Select OK. Add Sticky Notes

Sticky Note enables you to record patient-specific information, such as a future reminder to do a test, specific patient details to remember for the next visit, and more. The information does not print on the visit note document. Note: Recorded patient information from previous visits remains until manually erased. However, the database audit retains all patient information even after manual deletion. Open NextGen® Enterprise EHR, and select a patient from the Patient Lookup window. Select an encounter. On the Quick Buttons panel, select Sticky Note.The Patient Information window opens. In the Comments section, enter your notes, and then select Save & Close.The Sticky Note button changes to indicate that the selected encounter has saved the patient information. The Sticky Note supports up to 1000 characters. To view or edit saved patient information on an encounter, on the Quick Buttons panel, select Sticky Note.
