Significant Events Reference List

You can view reports of the significant events in NextGen® Enterprise PM and NextGen® Enterprise EHR. The reports that display significant events in NextGen® Enterprise PM are Significant Events and Significant Event History.

You can set up the following significant events in NextGen® Enterprise PM and NextGen® Enterprise EHR. The events are categorized by enterprise, practice, and system levels.

The following table describes the Enterprise Level Significant Events.

Address Changed through "Change of Address Update" ProcessTracks address changes that occur due to the Change of Address Update process (InNextGen® Enterprise PM,Filemenu >Processes>Change of Address Update).
Client Defined <1-14>Tracks changes made to the 14 client defined fields in theModify Patient/Person Informationwindow on theClient Definedtab. The event logs the client defined field name and its previous and current values for theSignificant Eventsreport. In the patient chart, the event is logged as<Field Name> from: <Previous Setting> to: <Current Setting>on theClinical History/Notestab >Chart Notes>History>Chart.
Demographics updated by External MPI systemTracks demographic changes that occurred due to updates by an external MPI system.
Duplicate Person Screen BypassedCreates a person-level significant event when a user does any of the following on thePotential Duplicate Personwindow.
  • Selects theIgnorebutton, and then selectsOKon the confirmation message.
  • Closes thePotential Duplicate Personwindow by selectingXon the upper-right corner or by another method, but then selectsOKon the message to confirm the changes for the duplicate person. If the user selectsCancelon the message, the person and significant event are not created.

  • Selects theOverridebutton to override the security that prevents some users from creating duplicate persons. This feature is controlled by the Create Duplicate Persons operations-level security right.

The significant event tracks the names of the person created and the user who created the person. When a user overrides the security, the event adds the textThe duplicate person screen was bypassed by overriding the security: <Last, First Names of the overriding user>in the Pre Mod column of theSignificant Eventsreport. The Pre Mod column is blank if user selectsIgnoreorxbut still confirms the creation.

Enterprise Batch Grouping CompletedTracks when batch groupings are completed. When the last practice batch is posted inNextGen® Enterprise PM, the enterprise grouping is set to completed, and it is removed from view.
Enterprise Batch Grouping CreatedTracks when users create batch groupings. The significant event includes the practice that the grouping was created in and the user who created it.
Enterprise Batch Grouping DeletedTracks when users delete batch groupings.
Merge PeopleRecords a person merge when the merge type is set to Enterprise and the database contains only one enterprise. You can view the event inNextGen® Enterprise PMin the following locations. In the Patient Chart >Clinical History/Notestab >Chart Notes>History>Encounters.

Page DeletedCreates aNextGen® Document Managementrelated significant event when a page from a document is deleted. The Page Deleted significant event appears only on theSignificant Eventsreport.
Page HiddenCreates aNextGen® Document Managementrelated significant event when a page is hidden from a document. The Page Hidden significant events appear only on theSignificant Eventsreport.
PHI Field ModifiedTracks changes inNextGen® Enterprise PMandNextGen® Enterprise EHRto any Patient Health Information fields on thePerson/Patient Informationwindow >Demographicstab and the UDS fields on thePatient/Person Informationwindow >UDStab. The event is recorded as<Field name> Changedon the PM Patient Chart >Clinical History/Notestab >Chart Notes>History>Chartand on theSignificant Eventsreport. The report also tracks the previous and current selections for the fields. TheCommunity Cd,Comment, andGenerate System Alertfields on theDemographicstab.

Practice Batch AddedCreates aNextGen® Document Managementrelated significant event when a practice batch is added.

Appointment AccessedLogs each time a user opens an existing appointment. The event logs the appointment date and time and patient name.
Appointment Conflict OverrideLogs each time a user successfully overrides a scheduling conflict.
Appointment Letters PrintedTracks when appointment letters are printed.
Appointment Waitlist Completed SuccessfullyTracks when an appointment waitlist item has been completed.
Appointment Waitlist CreatedTracks when an appointment waitlist has been created.
Appointment Waitlist DeletedTracks when an appointment waitlist has been deleted.
Balance Control ChangeTracks any time a user accesses the Balance Control window and modifies the charge buckets or the rebill indicators. Batch Account Collection Letters PrintedTracks when account-level collection letters are printed.
Batch Encounter Collection Letters PrintedTracks when encounter-level collection letters are printed.
Budget Letters PrintedLogs when a budget letter is printed, the person or patient on the budget plan, and the letter type. If a "Normal" budget letter is printed, the event includes the old and new due dates.
Budget Payment PostedLogs when payments are posted to the budget and the number of remaining payments.
Budget Plan DisabledLogs the disabled budget plan and the reason it was disabled.
Budget Plan EstablishedLogs new and re-established budget plans and their terms.
Budget Plan TerminatedLogs when a budget plan is terminated.
Budget Print Date ResetTracks when theResetbutton was last selected. SelectingResetenables you to reprint a batch of budget letters/statements from a specific date.
Budget Statement PrintedTracks when a budget statement has been printed.
Charge Credit Date UpdatedTracks when the credit date is updated. Credit date is determined when a line item reaches a negative balance. The event logs the previous value, the new value, and the amount of the credit.
Charge Field ModifiedLogs changes to the following Charge Posting fields for charges being applied to encounters and invoices.

Charge Outsource Status ResetLogs when an outstanding outsourced charge has been reset and is no longer being outsourced to a collection agency. The event logs the patient name and the associated encounter and SIM for theSignificant Eventsreport. In the patient chart, the event is logged asCharge Outsource Status Rest - Encounter: <Enc #> Charge SIM: <SIM code>on theClinical History/Notestab >Chart Notes>History>Encounters.
Charge OutsourcedLogs when an outstanding charge has been outsourced to a collection agency. The event logs the patient name and the associated encounter and SIM for theSignificant Eventsreport. Chart AccessedTracks when a user opens a patient chart inNextGen® Enterprise PM.
Chart Accessed in EHRTracks when a user opens a patient chart inNextGen® Enterprise EHR.
Chart CreatedLogs when a patient chart is created. The event logs the patient name, the user who created the chart, and the creation date.
Chart Created in EHRTracks when a new chart is created inNextGen® Enterprise.
Chart Guarantor ChangedTracks changes to the guarantor on the patient chart >Relationship/Contact/Guarantortab and records the event in the Patient Chart on theClinical History/Notestab >Chart Notes>History>Chart.
Chart UD1-8These eight Chart UD significant events track when changes are made to the chart user-defined fields. The names of the significant events match the client-defined names entered in Chart Practice Preferences. The event tracks the previous and current values of the fields and is logged as:Chart UD# from: <none> to: <Field Name>.
Claim CreatedLogs when a claim was created, when batch claims are printed, whether the claim was clean or dirty, the payer, and the media type.
Claim Deleted From EncounterClaim suppressed due to payer master indicator
Claim suppressed due to payer master indicatorTracks when a claim is suppressed due to a payer master indicator.
Copy of Archived Claim PrintedA practice-level significant event is recorded when archived claims are reprinted. This information is recorded only when the Actual Claim Form is selected on theClaim Printingwindow.
Credit Card Credit ProcessedTracks when a credit card payment is credited. Tracking information includes transaction date, active card type, last four digits of the card number, and expiration date.
Credit Card Sale ProcessedTracks when a credit card payment is completed. Tracking information includes transaction date, active card type, last four digits of the card number, and expiration date.
Credit Card Void ProcessedTracks when a credit card payment is voided. Tracking information includes transaction date, active card type, last four digits of the card number and expiration date.
Credit Card on File EnabledTracks when a credit card becomes the active card on file. Tracking information includes card type, last four digits of the card number, date the card became active, and expiration date.
Credit Card on File DisabledTracks when a credit card becomes disabled or is no longer the active card on file. Tracking information includes card type, last four digits of the card number, date the card is disabled, and expiration date.
Custom Dunning Message ChangedTracks when a custom dunning message has been changed.
Diagnosis ResolvedTracks when a patient diagnosis is resolved.
Document DeletedTracks when a document has been deleted.
Document ExportedTracks when documents are exported. You can view the event on theICS Sig Eventreport or by right-clicking an image and selectingImage Properties.
Eligibility Verification IssuedLogs when an eligibility verification is submitted fromNextGen® Enterprise PM.
Eligibility Verification Response ReceivedLogs the results of an eligibility submitted using NextGen Eligibility Verification.
Employer Insurance ChangeLogs when the insurance that is associated with a patient's employer has been changed on theEmployer Maintenancewindow. The event tracks the patient name, previous insurance, and current insurance for theSignificant Eventsreport. In the patient chart, the event is logged asEmployer Insurance Changeon theClinical History/Notestab >Chart Notes>History>Chart.
Encounter AccessedLogs each time a user opens an existing patient encounter. To view the significant event record, open the patient Account Profile and then select theClinical History/Notestab >Chart Notes>History>Encounters. The event is logged asEncounter Accessed - Encounter: <Enc #>.
Encounter Charge DeletedTracks when encounter charges have been deleted.
Encounter Collection Agency ChangeTracks when the collection agency is changed for an encounter in “bad debt” status in theAgency Bad Debt Maintenancewindow.
Encounter DeletedLogs encounters deleted from the patient chart and the associated encounter numbers. To view the significant event record, select the Patient Chart and then selectClinical History/Notestab >Chart Notes>History>Chart. The event is logged asEncounter Deleted - <Enc #>.
Encounter Field ModifiedCreates encounter-level significant events when users make changes to theEncounter Maintenancewindow >Billable Date,Billable Time,Occurrence Code,State,Onset Date, andOnset Timefields and to any fields on theGeneraltab.
Encounter Guarantor ChangedTracks changes to the guarantor on the patient chart >Encounterstab >Generalsubtab and records the event in the Patient Chart on theClinical History/Notestab >Chart Notes>History>Encounters.
Encounter Insurance DeletedTracks when a user deletes an insurance from an encounter on the Patient Chart >Encounterstab >Insurancesubtab. The event appears on theBalance ControlandEncounter Insurance Selectionwindows asEncounter Insurance Deleted: <insurance name> - Encounter: <encounter number>.
Encounter Insurance Re-OrderedCreates a practice-level significant event to track when an encounter insurance is reordered through Balance Control

when encounter insurance is added or removed on the encounter level (Patient chart >Encounterstab >Insurancesubtab >New)

when an insurance is deleted on the encounter level (Patient chart >Encounterstab >Insurancesubtab >Delete) .

The event appears on theBalance ControlandEncounter Insurance Selectionwindows asEncounter Insurance Re-Ordered - <encounter number>.

Encounter LockedTracks when an encounter has been locked.
Encounter Rate Adjustment ReversedTracks when encounter rate auto adjustments are reversed from an encounter.
Encounter Rate SIM and Amount RemovedTracks when an Encounter Rate SIM code is removed from an encounter
Encounter Status ChangeRecords the following status changes and the processes that generates them: Billed – Claim requested, statement printed

Bad Debt – Process through the menu

History – All charges that have no balance

In Progress to Unbilled – Process that converts an encounter status from "In Progress" to "Unbilled"

Rebill – Manually processed through Balance Control

Encounter UnlockedTracks when an encounter has been unlocked.
Form Template PrintedA significant event is recorded when a form is printed from any one of the following areas of the application: Patient Chart, Encounter Lookup, Appointment List, Appointment Book, Charge Posting, Payment Entry, Worklog Manager, and Account Lookup. The significant event can be tracked and viewed at the account, chart, and encounter levels.
Insurance Not Confirmed During AutoflowTracks when a user does the following actions during Autoflow checkout:
  • Does not selectYesto confirm insurance on theEncounter Insurance Selectionwindow.
  • Skips insurance confirmation by either selecting theCancelbutton on theEncounter Insurance Selectionwindow or by manually navigating to another step in the Autoflow checkout process.
Invoice Charge DeletedTracks when an invoice charge has been deleted.
Invoices PrintedTracks when an invoice has been printed.
Label Template PrintedA significant event is recorded when a label is printed from any of the following areas of the application: Patient Chart, Encounter Lookup, Appointment List, Appointment Book, Charge Posting, Payment Entry, Worklog Manager, and Account Lookup. The significant event can be tracked and viewed at the account, chart, and encounter levels.
Medical Necessity CreatedTracks when Medical Necessity requests are submitted. To view the significant event record, select the patient chart and then selectClinical History/Notestab >Chart Notes>History>Encounters. Medical Necessity tracking records the date and time, creator, and a description for each Medical Necessity query.
Medication RenewedTracks when a medication has been renewed for a patient.
Medication StopTracks when a medication has been stopped for a patient.
Merge AccountsRecords the account merge when a person is merged. You can view the event inNextGen® Enterprise PMinAccount Profile>Notes>History.
Merge ChartsRecords the chart merge when a person is merged. You can view the event inNextGen® Enterprise PMinPatient Chart>Clinical History/Notes>Chart Notes>History.
Merge EncountersRecords all merged encounters that occur during a person merge. You can view the event inNextGen® Enterprise PMinPatient Chart>Clinical History/Notes>Chart Notes>History>Encounters.
New Encounter CreatedTracks when a new encounter has been created. The event logs the encounter number, patient name, the user who created the encounter, and the creation date.
Page AnnotatedCreates a significant event when annotations to a page are saved inNextGen® Document Management. Additionally, annotations on aNextGen® Document Managementimage inNextGen® Enterpriseare tracked when a signature is added or the image is rotated.
Page AppendedCreates aNextGen® Document Managementrelated significant event when users add a page to a filed document. To view the significant event record, select the patient chart and then selectClinical History/Notestab >Chart Notes>History>Document Management. The significant event record also appears on theImage Propertieswindow inNextGen® Document Management.
Page FiledCreates aNextGen® Document Managementrelated significant event when users file document pages inNextGen® Document Management. To view the significant event record, select the patient chart and then selectClinical History/Notestab >Chart Notes>History>Document Management.

Patient Allergy DeletedCreates a practice-level significant event to track when an allergy is deleted from a patient's allergy list.
Patient Procedure Activity On a Locked EncounterTracks when a user creates, updates, or deletes a charge withinNextGen® Enterprise PMor other external interface and the change automatically updates the patient procedures inNextGen® Enterprise EHRwhen the encounter is locked inNextGen® Enterprise EHR. The significant event tracks the encounter, service item number, and date of service asA patient procedure was inserted onto a locked encounter. Encounter: <number>, service item id: <number>, date of service: <mm/dd/yyyy>.
Patient Status ChangeLogs when a user changes a patient status on thePatient Informationwindow >Statustab. The event logs the patient name and the previous and current statuses for theSignificant Eventsreport. In the patient chart, the event is logged asPatient Status Change from: <Previous Status> to: <Current Status>on theClinical History/Notestab >Chart Notes>History>Chart.
Privacy Notice Information ChangeTracks changes to Privacy Notice information as a Significant Event. Privacy Notice tracking records the date and time, creator, and a description for each change made to Privacy Notice information. To display the significant event record, select the Patient Chart and then selectClinical History/Notestab >Chart Notes>History>Chart.
Real-time Adjudication Request SubmittedTracks when real-time adjudication requests have been submitted.
Recall Letters PrintedTracks when recall letters have been printed.
Recall Print Date ResetRecall Print Date Reset significant event is recorded in each patient's chart. When a batch of recall letters is printed/exported and the Updated Dates button is selected, each patient's recall plan is updated with a 1st Mailing Date, 2nd Mailing Date or 3rd Mailing Date. At that point, the recall plans in the batch no longer qualify for a letter.
Re-establish Budget PlanTracks when a budget plan has been re-established.
Referral Request IssuedTracks when referral request responses have been issued.
Referral Request Response ReceivedTracks when referral request responses have been received.
Relationship DeletedTracks when relationships are deleted from the patient chartRelationship/Contact/Guartab.
Relationship HiddenTracks when relationships are hidden in the patient chartRelationship/Contact/Guartab.
Reviewed Document SignedCreates aNextGen® Document Managementrelated significant event.
Started For The First TimeTracks when aNextGen® Document Managementprocess has been started for the first time.
Statement Counter ResetCreates a practice-level significant event when the statement counter is reset to zero on theAccount ProfilePropertiestab. The event states the old and new values.
Statement Print Date ResetIndicates the statement print date was reset so statements can be regenerated from a specific date.
Statements PrintedTracks when statements are printed in batch or on-demand and the user answersYesto the prompt "Did the statements print correctly?" The event tracks last "next statement" date, new next statement date, demand or batch mode, and whether "Generate Statement Next Run" is on or off.
Task Field ModifiedCreates chart-level significant events when users make changes on theAdd/Edit Taskwindow >Task Infotab and theTask Approvaltab. The event and the old and new settings are recorded on theAdd/Edit Taskwindow >Historytab.
Unposted Transaction DeletedTracks when an unposted transaction is deleted from a batch. If a credit card was used for the transaction, the significant event record includes the last four digits of the credit card and the TransFirst® transaction number.
Unposted Transaction ModifiedTracks when any of the following fields are modified for an unposted transaction in Payment Entry:
  • Tracking description
  • Adj Amt
  • Pay Amt
  • Pay Code
  • Adj Code
  • Clm Reasons
  • Ln Item Rsns
  • Deduct
The event is logged asUnposted Transaction Modified Batch Date: <mm/dd/yyyy> Batch Nbr: <xxxx> Batch Item Nbr: <xxxx>on theClinical History/Notestab of the patient chart.

As it tracks who changes the transactions, this significant event can help to prevent fraud.

Page ImportedCreates a system-level significant event when users perform a file, page, or directory import. This event does not track auto directory import. The significant event record appears on theImage Propertieswindow inNextGen® Document Management.
Page ScannedTracks when users scan a page. This significant event appears only on theSignificant Eventreport.
Patient Discount <Name of Discount> CreatedReserved for future use.
Patient Discount <Name of Discount> Deleted (WIP)Reserved for future use.

Person/Patient Lookup CriteriaDisplays the patient records that users have searched for. If the event is enabled, and when you search for a person or patient, the application records the search criteria. For example, if you search for John Doe with a birthdate of 01/01/1950, the following event is recorded:(Last): Doe, (First/Preferred): John, (DOB): 01/01/1950
Reports Ran By UserCreates a practice-level significant event to track the details of report generation every time a user runs a report. The Significant Event message contains the following information: User who ran the report.

Date and time that the report completed.

The Filter Criteria used to run the report (as printed in the report footer) displays in thePost Modcolumn.

Security Item ModifiedTracks security rights that are modified for user groups in System Administrator. The information is recorded as a system-level significant event in theSecurity Item Modified, Group: <group name>format, along with whether it is anOPERATIONSorMODULESright, the security right name, its previous and current settings, and the user who made the change.
Service Item ModifiedTracks all pricing updates made to SIMs that result from SIM Pricing Templates. The details that this significant event logs includes the SIM Library Configuration applied, who performed the update, and when it occurred. In addition to updates initiated by users, the tracking includes changes made by SIM Pricing Template Updates inNextGen® Background Business Processor.

Valid LoginTracks each time a user logs on to anyNextGen® Enterpriseapplication.
Valid Logout -NextGen® Enterprise EHRTracks each time a user logs offNextGen® Enterprise EHR.
