Assessment and Plan Comments

Use the Comments (Assessments) and Comments (Plan) fields to record observations, additional information, instructions and plans that do not have discrete data fields for that purpose. You can type text into the field or use saved phrases from a “My Phrases” set. See Add Comments. The Review Comments link above the field opens a window where you can review comments entered in all sections of the patient’s chart.
