Document Details of an Unlisted HPI or Reason for Visit
To document details of a Reason for Visit that is not shown in the left-side navigation, select the blue down-arrow to the right of the
Other
field.
A picklist opens and lists templates to Reasons for Visit or HPI.
Select a template, and then select
OK
to open it.
Make selections and enter text to capture the relevant details for the HPI.
Select
Save & Close
to enter the HPI in the
Reason for Visit / HPI
grid.

Add Additional Reasons for Visit Not Listed
Select the Additional/Manage link below the Other field in the left-side navigation. The Reason for Visit window opens. Reason for Visit window Select the reasons from the list on the left. As you select reasons, they enter the fields to the right. For any reasons that you want to follow up on, select the Follow up checkbox above the field for the reason. Select Save & Close. Delete Reasons for Visit Not Listed

Add Complaint Details

Document Reasons for Visit or HPI
Use the grid and left-side navigation to include Reasons for Visit and HPI details. You can add up to six. These can be simple listings of complaints or full details including onset, severity, aggravating and alleviating factors. Reason for Visit panel on the Chiro Single Visit template If you only want to enter the complaint without details, select the Do not launch HPI checkbox. Then, when you select items from the left-hand navigation, the templates do not open, but the general descriptions enter the grid. You can also: Add reasons for visit that are not listed as left-side navigation links, both with details (captured on a template) or without. Open a template to add details to a Reason for Visit or HPI that is already in the grid. Delete a Reason for Visit or HPI that is already in the grid. Document the Reason for Visit
The Reason for Visit panel is where the reason for visit, as described by the patient, is documented, along with an initial History of Present Illness (HPI). The provider should review reasons for visit, update if necessary, and complete HPI information. Reason for Visit If the Introduction field contains text, review the content and make any necessary changes. The introduction appears as the first sentence of the visit note, and it can be edited for the individual patient. A typical introduction might contain the patient's age, gender, and the reason for visit. If the introduction field is blank, compose an introduction for the patient. If checked, click to clear the Do not Launch HPI check box. In the Reason for Visit data grid, click one of the reasons for visit entered previously to open the HPI template to add additional details. Patient Reasons for Visits
You can document a patient's reasons for visit. On the Reason for Visit panel, you can document the common reasons for a patient visit. You can view the configurable specialty-specific reasons for visit list on the left pane. When you select a reason for visit that is already selected in the current encounter, a message appears highlighting the duplication. You can retain or remove the duplicate reason for visit. You can select up to 14 History of Present Illness (HPIs) simultaneously. Additionally, you can select the Do not launch HPI check box and any HPI to open the corresponding HPI template. Note: You can configure submenu items for the reason for visit on the Framework Content system template for the specialty. Based on the configuration, on the applicable template, the submenu items appear in the Reason for Visit panel on the left pane under the respective HPI.
