Select the complaint in the grid to open the template.
Make all appropriate selections and entries.

Document Details of an Unlisted HPI or Reason for Visit
To document details of a Reason for Visit that is not shown in the left-side navigation, select the blue down-arrow to the right of the Other field. A picklist opens and lists templates to Reasons for Visit or HPI. Ngkbm Udg Rfv Picklist Select a template, and then select OK to open it. Add Additional Reasons for Visit Not Listed
Select the Additional/Manage link below the Other field in the left-side navigation. The Reason for Visit window opens. Reason for Visit window Select the reasons from the list on the left. As you select reasons, they enter the fields to the right. For any reasons that you want to follow up on, select the Follow up checkbox above the field for the reason. Select Save & Close. Document Reasons for Visit or HPI
Use the grid and left-side navigation to include Reasons for Visit and HPI details. You can add up to six. These can be simple listings of complaints or full details including onset, severity, aggravating and alleviating factors. Reason for Visit panel on the Chiro Single Visit template If you only want to enter the complaint without details, select the Do not launch HPI checkbox. Then, when you select items from the left-hand navigation, the templates do not open, but the general descriptions enter the grid. You can also: Add reasons for visit that are not listed as left-side navigation links, both with details (captured on a template) or without. Open a template to add details to a Reason for Visit or HPI that is already in the grid. Delete a Reason for Visit or HPI that is already in the grid. Add Additional History
To add additional history, enter it in the Additional History field. Additional History field on the Billable Visit template You can type in the field, click the down arrow to use My Phrases, or click the check mark to use dictation software to enter information in below the grid. Add Details to a Complaint
When capturing specifics about an individual complaint, be sure that the option next to that complaint is selected. Select the checkboxes to the right of the Patient Complains of fields to add details about the complaint. Document specifics only for that complaint. Fields for patient complaints As needed, select the No or Yes option for any Recent Changes in the patient’s medical status or Medications. If there are changes, you can free type details into the fields. Select the Add button to save the information to the History of Complaint(s) grid. Parent topic: Chief Complaints

Add Complaints to the List
Select in the white field below Patient Complains of. The picklist of complaints open. Select an item from the picklist or select the blank at the top of the picklist to type into the field. Chief Complaint picklist When you have entered a complaint, it moves to one of the blue fields, and the option for the next blank field is selected. You can then either add another complaint following steps 1 and 2 above, or you can add details to the complaint and move it to the grid. Parent topic: Chief Complaints
