Past Medical and Surgical History

The Past Medical History grid displays any Medical and Surgical history that was captured in a NextGen Healthcare application in your organization or practice. For example, if you have one practice but multiple locations, all locations can see the Past Medical History regardless of where it was entered. Add Surgery Information to the NTS Homepage Template

You can document the patient's past and upcoming surgeries and indicate whether they pertain to the injury you are treating. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Homepage template, and then select a visit type. Below the Surgery grid, select Add to open the NTS Surgery template. Select Surgery Description, and then do one of the following in the Surgeries list, do one of the following: Select a surgery name, and then select OK. Select the first blank line, select OK, and then enter a custom description. Enter the surgery date and CPT4 code. Select Add. In the confirmation message, do one of the following: If the surgery is related to today’s treatment, select Yes. The surgery information appears in both the Surgical History and Surgery Related to Therapy Episode grids. If the surgery date occurs after the patient completes treatment, the numbers in the Days and Weeks columns appear as negative numbers. If the surgery

Indicate a Surgery Is Related to the Patient's Treatment

Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Homepage template, and then select a visit type. Below the Surgery grid, select Add. The NTS Surgery template opens. Select a surgery listed in the Surgical History grid. Select Related to Tx. The surgery is added to the Surgery Related to Therapy Episode grid. Select Close. Add Treatment Plan Details for Audiology Visits

In the NTS Audiology Single Visit template, expand the Plan panel, and then go to the Treatment Plan section. If this is the first visit in this case, the Treatment grid will be empty. Afterwards, when you open this template, all entries in the grid copy forward from the prior visit. Select a category of treatment activities by doing one of the following: To enter general treatment activities, select Default. To enter treatment activities specific to a specialty, select a specialty category. Note: Administrators can configure categories and associated picklists in the NTS Picklist Manager practice template. Select the Treatment field. A picklist of treatment items opens. Do one of the following to add all treatment activities you plan to use to rehabilitate the patient: Select the items from the picklist, and then select Close or Done. As you select each item, it automatically appears the Treatment grid. To enter a treatment that is not listed in the picklist, select the blank space in

Manage Surgical History of Patients

On the appropriate template, in the Surgical History or Orthopedic Surgical History section, select the procedure to be edited or removed from the grid. Surgical History grids To edit the selected procedure, do the following: Click Edit below the corresponding grid. The IORTS Px Edit Detail window opens. IORTS PX Edit Detail window Make the desired changes. Note: The CPT4 field is read-only. The Global Date field is automatically filled based on the surgery date for the procedures documented in the patient history sections on the Surgery Coding, Single Fx, and Multi Fx templates. Click Update. The updated details appear in the grid. To remove the selected procedure, click Remove below the corresponding grid, and then select Added in Error from the list. The procedure is removed from the grid. Note: You must be on the encounter where the procedure is added, to remove the selected procedure. Add Medical History

You can add medical history such as diseases, disorders, and surgical procedures of a patient. On the appropriate template, in the Medical History section, click Add. The Past Medical History template opens. Past Medical History template Note: By default, the Specialty field displays Orthopedics. In the Medical and Surgical panels, do the following steps for the options without a link. Select the desired check box. The details Details icon icon and a blank date field appear. Click the date field. The Date Selection window opens. Click the calendar icon, and select the date when the disease or disorder first appeared or when the surgery is performed for the patient. Note: You can also enter the date in the field. To add the documented details to the grid in the Past Medical History panel, click Add To Grid.
