Add Surgery Information to the NTS Homepage Template
You can document the patient's past and upcoming surgeries and indicate whether they pertain to the injury you are treating. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Homepage template, and then select a visit type. Below the Surgery grid, select Add to open the NTS Surgery template. Select Surgery Description, and then do one of the following in the Surgeries list, do one of the following: Select a surgery name, and then select OK. Select the first blank line, select OK, and then enter a custom description. Enter the surgery date and CPT4 code. Select Add. In the confirmation message, do one of the following: If the surgery is related to today’s treatment, select Yes. The surgery information appears in both the Surgical History and Surgery Related to Therapy Episode grids. If the surgery date occurs after the patient completes treatment, the numbers in the Days and Weeks columns appear as negative numbers. If the surgery

Add Surgery Information to an Audiology Visit
You can document the patient's past and upcoming surgeries and indicate whether they pertain to the injury you are treating. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Audiology Single Visit template, and then select a visit type. In the Subjective panel, select Surgery Hx to open the NTS Surgery template. The Surgical History grid displays any documented past surgeries for the patient. The Surgery Related to Therapy Episode grid displays surgeries related to the current episode of care. Select Surgery Description, and then do one of the following in the Surgeries list: Select a surgery name, and then select OK. Select the first blank line, select OK, and then enter a custom description. Enter the surgery date and CPT4 code. Select Add. Homepage Template
Open the NTS Homepage template as the starting point for all visits. On it, you can view key information such as scripts, authorizations, KX modifier, and the plan of care. The application tracks this information and, when you open the NTS Homepage template, alerts you if action is needed. Add Medical and Surgical History
You can add the medical and surgical history of a patient on the Histories template or the History Summary panel. You can add medical and surgical history information in the History Summary section. If there is no relevant history information for a patient, you can select No relevant past medical/surgical history. Open NextGen® Enterprise EHR. Open the Intake template. In the History Summary section, select Past Medical/Surgical. Select Add Past Medical/Surgical Hx. Document the reported history information in the corresponding section. Select Add To Grid, and then select OK.The history records appear in the Past Medical History and Past Surgical History grids. Historian
You can record if someone other than the patient is providing information on the patient’s medical history. The entry in the Historian field also appears on the Office Visit document. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Homepage template associated with the encounter, and then select a visit type. Select the Historian field, and then do one of the following: Select the relationship of the historian to the patient. Select a Visit Type for the Patient Encounter
The visit type you select determines the workflow for the patient encounter. Note: In the Billing Configuration system template, your administrator configures which visit types you can choose from. For more information, go to NextGen Healthcare Success Community, and download the latest Configuration Guide for NextGen® Therapy Suite. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Homepage template associated with the encounter.
