Past Medical History

Any past medical history entered in the NextGen® Enterprise EHR appears in the Past Medical History grid. You can add additional items to it from the NTS Sp PMH template. You can also add family and social history for the patient, but those do not appear in the grid. Document Alcohol and Caffeine Use

You can document alcohol and caffeine consumption for the patient. Select Alcohol/Caffeine on the side navigation. Enter information about the patient’s alcohol and caffeine use in the applicable fields. Social History template with Alcohol/Caffeine documented For the question Do you drink alcohol?, if you select the Yes or Formerly option, additional fields open so you can enter information on type and amount of alcohol. Document Other Social Histories

You can enter Statuses, Lifestyle, Occupation, Comments, Diet History, Pediatric/adolescent social History, and Environmental histories. Select the appropriate template from the side navigation. Enter applicable information into the fields. Select Save & Close. You can document alcohol and caffeine consumption for the patient. Note: If alcohol use was a problem for the patient, you can enter that on the Confidential History template. Click Alcohol/Caffeine on the side navigation. Social History - Alcohol/Caffeine window Enter information about the patient’s alcohol and caffeine use in the applicable fields. Family History

You can review, add, or edit family history for the patient in the Family History panel. You can document past family history on the Family Health History template by selecting the Add button in the Family panel. The Family Health History template includes individual and grouped conditions or diseases. You can document medical conditions that family members have been diagnosed with, along with the age of onset and whether the diagnosis was the cause of death on the Family Health History template. You can also document that there is no family history of a disease or disorder by selecting the No family history of check box. Add Confidential History

You can enter confidential history for social history problems (Alcohol or Drug use abuse, Psychiatric, Abuse or Incarceration, sexual practices or STI’s) for a patient. Click the Confidential History link below the Social History grid. Confidential- Alcohol/Drug template The Confidential- Alcohol/Drug template opens. Enter patient-specific data in the applicable fields. Use the side navigation to navigate to templates for other confidential history. To include confidential histories with patient consent on the documents, select the Include all confidential information in the social history document with patient permission check box.
