Document Alcohol and Caffeine Use

You can document alcohol and caffeine consumption for the patient. Select Alcohol/Caffeine on the side navigation. Enter information about the patient’s alcohol and caffeine use in the applicable fields. Social History template with Alcohol/Caffeine documented For the question Do you drink alcohol?, if you select the Yes or Formerly option, additional fields open so you can enter information on type and amount of alcohol. Document Other Social Histories

You can enter Statuses, Lifestyle, Occupation, Comments, Diet History, Pediatric/adolescent social History, and Environmental histories. Select the appropriate template from the side navigation. Enter applicable information into the fields. Select Save & Close. Histories Template

Patient history information is documented on the Histories template for a new patient, and reviewed and updated for an existing patient. Histories Patient's Problem List You can manage a patient's problems. Patient's Medical History The Medical/Surgical/Interim panel allows you to document the medical or surgical history of the patient. Patient's Diagnostic Tests and Results You can document a patient's diagnostic tests and the associated results. Patient's Family History You can document the family history of the patient in the Family panel on the Histories template. Histories Template

The Histories template enables you to document a patient's social history, problem list, medical or surgical history, diagnostic study history, and family history. The panels available in the Histories template are as follows: Table 1. Section Description Problem List You can view, add, modify and retrieve patient's clinical problems and chronic conditions. Medical/Surgical/Interim You can document the patient's problem list and social, medical or surgical, diagnostic study, and family histories. Diagnostics Studies You can document the patient's diagnostic tests and its associated results. Family You can document the family history of the patient. Social You can document the social history of an adult or a pediatric patient. The categories in the social history includes tobacco/vaping use, alcohol/caffeine, lifestyle, occupation and so on. Categories in Social History

You can use different sections in the Social history to document the history of a patient. The following categories are available in the Social history. Category Description Tobacco/Vaping Use Enables you to document the tobacco usage for a patient. Includes the attempts to quit, second-hand (passive) tobacco exposure and to screen the patient for vaping, and document additional vaping details. Alcohol/Caffeine Enables you to document the alcohol and caffeine use for the patient. Statuses/General Enables you to document the patient's general demographic information including race, ethnicity, preferred language, country of birth, educational levels, employment, military experience, social support, and housing. The Current status field is read-only and displays the details documented on the Update Patient Information template. Lifestyle Enables you to document the lifestyle details.
