You can enter Statuses, Lifestyle, Occupation, Comments, Diet History, Pediatric/adolescent social History, and Environmental histories. Select the appropriate template from the side navigation. Enter applicable information into the fields. Click Save & Close. Add Family/Social History

Select Fam/Soc Hx. Fam/Soc Hx button under Past Medical History section A picklist opens for you to select the type of history you want to document. Select the history, and then select OK. The Social History template opens to the page you selected. You can select other social histories from the left navigation pane. Documenting a Patient's History Summary

You can add, update, delete, and review a patient's history summary details from the History Summary panel. You can document a patient's history details in the History Summary panel in the following categories: Past Medical/Surgical Interim Social Family Diagnostic The History Summary panel also displays the patient history documented by the intake staff on the Histories template. You can add diagnostic studies with a documented Result or Interpretation to the grid in the Reason for Visit panel by selecting the Add to HPI Grid button. The grid includes a list of categories on the left pane to select and view the documented data. You can filter the patient history records by selecting the Medical History or Surgical History option for Past Medical/Surgical. You can view and enter confidential details provided by the patient during the course of the encounter using The Confidential link. Categories in Social History

You can use different sections in the Social history to document the history of a patient. The following categories are available in the Social history. Category Description Tobacco/Vaping Use Enables you to document the tobacco usage for a patient. Includes the attempts to quit, second-hand (passive) tobacco exposure and to screen the patient for vaping, and document additional vaping details. Alcohol/Caffeine Enables you to document the alcohol and caffeine use for the patient. Statuses/General Enables you to document the patient's general demographic information including race, ethnicity, preferred language, country of birth, educational levels, employment, military experience, social support, and housing. The Current status field is read-only and displays the details documented on the Update Patient Information template. Lifestyle Enables you to document the lifestyle details. Family History

You can review, add, or edit family history for the patient in the Family History panel. You can document past family history on the Family Health History template by selecting the Add button in the Family panel. The Family Health History template includes individual and grouped conditions or diseases. You can document medical conditions that family members have been diagnosed with, along with the age of onset and whether the diagnosis was the cause of death on the Family Health History template. You can also document that there is no family history of a disease or disorder by selecting the No family history of check box. Histories Template

The Histories template enables you to document a patient's social history, problem list, medical or surgical history, diagnostic study history, and family history. The panels available in the Histories template are as follows: Table 1. Section Description Problem List You can view, add, modify and retrieve patient's clinical problems and chronic conditions. Medical/Surgical/Interim You can document the patient's problem list and social, medical or surgical, diagnostic study, and family histories. Diagnostics Studies You can document the patient's diagnostic tests and its associated results. Family You can document the family history of the patient. Social You can document the social history of an adult or a pediatric patient. The categories in the social history includes tobacco/vaping use, alcohol/caffeine, lifestyle, occupation and so on.
