You can enter Statuses, Lifestyle, Occupation, Comments, Diet History, Pediatric/adolescent social History, and Environmental histories. Select the appropriate template from the side navigation. Enter applicable information into the fields. Click Save & Close. Add Family/Social History

Select Fam/Soc Hx. Fam/Soc Hx button under Past Medical History section A picklist opens for you to select the type of history you want to document. Select the history, and then select OK. The Social History template opens to the page you selected. You can select other social histories from the left navigation pane. Document Occupation of a Patient

You can document the occupation details of a patient in the Social History - Occupation template. Open NextGen® Enterprise EHR. To open the Social History - Occupation template, do one of the following: On the Histories template, expand the Social panel, select Occupation, and then select Add. In the History Summary panel on Intake, select Occupation from Social, and then select Add. Enter the employment and employment status in the corresponding fields. Enter phone number and restrictions in the corresponding fields. Select occupation from the list in the Occupation field. To clear and select another occupation, select Cl button and select occupation from the list. To select the retirement date, select the Retired field. Save and close the template. Add Social History

You can add the social history of a patient in the History Summary panel on the Intake template and in the Social panel on the Histories template. Open NextGen® Enterprise EHR. To document the social history details, do one of the following: In the History Summary panel on the Intake template, select Social, select the relevant option in the list and Add/Edit. On the Histories template, expand the Social panel, select the relevant option, and then select Add/Edit. The social history template opens. Document the reported history information. Patient's Tobacco Use Information You can document the patient's current or past tobacco use information. Categories in Social History

You can use different sections in the Social history to document the history of a patient. The following categories are available in the Social history. Category Description Tobacco/Vaping Use Enables you to document the tobacco usage for a patient. Includes the attempts to quit, second-hand (passive) tobacco exposure and to screen the patient for vaping, and document additional vaping details. Alcohol/Caffeine Enables you to document the alcohol and caffeine use for the patient. Statuses/General Enables you to document the patient's general demographic information including race, ethnicity, preferred language, country of birth, educational levels, employment, military experience, social support, and housing. The Current status field is read-only and displays the details documented on the Update Patient Information template. Lifestyle Enables you to document the lifestyle details. Family History

You can review, add, or edit family history for the patient in the Family History panel. You can document past family history on the Family Health History template by selecting the Add button in the Family panel. The Family Health History template includes individual and grouped conditions or diseases. You can document medical conditions that family members have been diagnosed with, along with the age of onset and whether the diagnosis was the cause of death on the Family Health History template. You can also document that there is no family history of a disease or disorder by selecting the No family history of check box.
