Document Alcohol and Caffeine Use

You can document alcohol and caffeine consumption for the patient. Note: If alcohol use was a problem for the patient, you can enter that on the Confidential History template. Click Alcohol/Caffeine on the side navigation. Social History - Alcohol/Caffeine window Enter information about the patient’s alcohol and caffeine use in the applicable fields. If you select the Yes option, additional fields open to enter information on type and amount of alcohol. Enter the details in those fields. Click Save & Close. Parent topic: Patient Intake Details

Add Family/Social History

Select Fam/Soc Hx. Fam/Soc Hx button under Past Medical History section A picklist opens for you to select the type of history you want to document. Select the history, and then select OK. The Social History template opens to the page you selected. You can select other social histories from the left navigation pane. Document a Patient's Alcohol Use

You can document the alcohol consumption details of a patient on the Confidential – Alcohol/Drug template. Open NextGen® Enterprise EHR. To open the Confidential – Alcohol/Drug template, do one of the following: In the Social panel on the Histories template, select the Confidential History link. In the History Summary panel on the Intake template, select the Confidential link. In the Alcohol Use section, enter the age at which the patient had started drinking alcohol. Indicate if the patient has sought treatment for alcohol abuse. If you select Yes, then do the following: Enter the date of the last treatment for alcohol abuse. Enter the number of treatments taken for alcohol abuse. Indicate if the patient is involved in a 12-step program. If you select Yes, select Currently or Formerly. Indicate if the patient has had withdrawal problems, seizures, or blackouts from alcohol or drugs. Indicate if the patient has required emergency medical attention due to intoxication. Categories in Social History

You can use different sections in the Social history to document the history of a patient. The following categories are available in the Social history. Category Description Tobacco/Vaping Use Enables you to document the tobacco usage for a patient. Includes the attempts to quit, second-hand (passive) tobacco exposure and to screen the patient for vaping, and document additional vaping details. Alcohol/Caffeine Enables you to document the alcohol and caffeine use for the patient. Statuses/General Enables you to document the patient's general demographic information including race, ethnicity, preferred language, country of birth, educational levels, employment, military experience, social support, and housing. The Current status field is read-only and displays the details documented on the Update Patient Information template. Lifestyle Enables you to document the lifestyle details. Document a Patient's Alcohol Use

You can document the alcohol consumption details of a patient on the Confidential – Alcohol/Drug template. Open NextGen® Enterprise EHR. Open the Histories template. Document Current Tobacco Use

Expand the Tobacco Use panel. If the patient currently uses tobacco, select the Yes option next to Have you ever used tobacco. If the patient: Smoked tobacco in the last 30 days, select the Yes option next to Have you used tobacco in the last 30 days, and then record the types, quantities, and frequencies under the Smoking Tobacco Use section. Chews tobacco or takes snuff, select the Yes option next to Have you used smokeless tobacco in the last 30 days, and then record the types, quantities, and frequencies under the Non-Smoking Tobacco Use section. Do one of the following: Select OK. To document additional tobacco history, expand other tobacco template panels as needed. To document another social history, select one from the side navigation.
