NextGen® PxP Portal Interactive Medical Forms

Interactive Medical Forms are decision-based questionnaires primarily designed to gather medical history and diagnostic information from a patient prior to a visit. Patients can complete and submit these forms online through NextGen® PxP Portal. Optionally, they can also print out, complete, and submit the forms when they visit the practice.
