Enter Vital Signs

You can enter vitals for today’s visit. Click the Add button next to the Vitals grid. Adult Vital Signs template The Vital Signs template opens. Enter measurements in the fields. Click Save to save data you have entered. Click Close. Note: Vitals and alerts are viewable on the Physical Exam sections of other workflow templates. The vitals are shown in the grid. Values outside the normal range trigger a red alert to appear above the grid. Parent topic: Vitals

Vitals

Vital signs information entered from the Intake template is shown in the Vitals field. Abnormal vital signs values are flagged with red alerts on the Physical Exam panel header. Vitals field on the Billable Visit template You can add or edit vital sign measurements in the Physical Exam panel, view graphs and historical vitals sign data, and access the Health Promotion Plan template. Vital Signs Graphs

You can view vital signs in graph format. Click the Graph link next to the Vitals grid. Adult Vital Signs Graphs template The Adult Vital Signs Graphs template opens, showing a graph of the patient’s blood pressure. Use the side navigation to view graphs of other vital signs. Document Historical Vital Signs

You can document the historical vital signs for a patient. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. Expand the Vital Signs panel and select Add. The age-specific vital signs template opens. Document the patient's recorded historical vital signs. In Weight measurement, document the patient's weight in the lb and kg fields within the 4.409 – 1543.3 lb or 2.00 – 699.99 kg range. Note: An alert message appears if the value entered is not within the range. In Blood Pressure and pulse, document the patient's Systolic and Diastolic readings within the 0 – 399 range, and then document the patient's pulse rate within the 10 – 499 range. Vital Signs

You can add, edit or remove vital signs for a patient using the Vital Signs panel. In the Vital Signs panel you can view vital signs documented during this encounter. You can edit ot remove previously entered vital signs and enter new vital signs. The Vital Signs panel is also available on other office visit templates. The Historical information entered this encounter link opens the Vital Signs History template filtered to display only vital signs documented with patient's encounter. Using the History link, you can review the patient’s entire vital signs history on the Vital Signs History template. Using the Graphs link, you can view vital signs in graphical format on the age-appropriate VS Charts template. Vital Signs The vital signs templates are based on age. The following buttons and links available in the Vital Signs panel. Button/Links Description Add Enables you to add new information. Edit Enables you to review and make the required changes to the documented vital signs. Remove Enables you to delete a vital sign record. History Enables you to view the vital signs details in a tabular format. Graph Opens graphical views of the data based on the specialty and age of the patient.
