Update a Medication Adherence

Reconcile Medications Manually
Use the Medications Review panel on the Medications Review template to reconcile medications manually. Note: If no manual reconciliation has taken place, the Manual Reconciliation check box in the Reconciliation type panel is unchecked; as soon as you reconcile any medication manually, the box is selected and fields open for you to enter additional data. Medication Review template Click the Reconcile button below the Medications grid to open the Medication Review template. If the patient is taking all medications in the Medication List grid as directed, click the Review All – Taken As Directed button to move them all into the Medication Review grid. If not, skip to step 3. Select the check box Review – adherence at the top of the Medication Review panel. The drop-down field selection defaults to “taking as directed”. In the Medication List, click on any medication that the patient is taking as directed. Medications that you select move to the Medication Review grid with that adherence

Reconcile Medications
The Medications Review template enables you to reconcile medications listed for the patient with the medications the patient is taking and how the patient is taking them. The Medications Review template has three panels: Reconciliation type – has a check box that indicates if reconciliation was completed manually and has an Electronic Reconciliation button. To use electronic reconciliation, you must have a CCDA document or medication record to import. Medication Module– displays all medications that were added from the Medications module. Medication Review– displays current medications pending review in the Medication List and reviewed medications appear in the Medication Review grid. Medications
Use the Medications grid to view the patient's active medications. These are pulled from the Medications module. You can reconcile the medications, add new ones, or remove ones that are no longer active. To indicate that medications have been reviewed, select the Reviewed check box. The None check box is automatically selected if no medications are listed. To remove a medication, select it and click Remove. Note: When you click Remove, it instantly removes the selected medication from the grid and changes its status to inactive in the Medications module. Make certain that you have selected the medication that you intend to remove. To add or edit a medication, click Add to open the Medications module. Reset Display of Medications List Columns
You can reset the display of Medications List columns to the default view. Right-click the header row of the Medications List, or select Grid Preferences. Select Reset Columns. Reset Columns Parent topic: Medications List

Remove an Allergy
You can remove an allergy and enter the reason for the removal. Select an allergy in the Allergies grid. Click Remove. Remove allergies from the Allergies grid Entered in error- the allergy was entered in error in the Medication Allergy module. Disproved- the allergy is disproved in the Medication Allergy module. Resolved- the allergy is resolved, with today’s date as the resolved date in the Medication Allergy module. The Reason picklist opens with a list of reasons for removing the allergy. Select a reason.
