Enter Attendant Care
Use the
Attendant Care
template to generate a note for any attendant care services.
Click the
Forms
link, and double-click
Attendant Care
from the list.
The
Attendant Care
template opens.
In the
Diagnosis
grid, select a diagnosis.
In the
Surgery
grid, select a procedure.
Select the attendant care services needed. Use the
Misc.
field to enter any types of care not specified.
Enter the date range and hours.
Record the
Signed Provider
.
Once all information is entered, select the
Done
check box.
Generate the document by clicking the
Attendant Care
document icon.

Complete the Replacement Services Form
Use the Replacement Services template to generate a note for any required replacement services. Replacement Services template Click the Forms link, and double-click Replacement Services. In the Diagnosis grid, select a diagnosis. In the Surgery grid, select a procedure. Select the desired replacement services. Use the Misc. field to enter any other types of care. Enter the date range and hours. Select the Signed check box and enter the Provider. Once all information is entered, select Done. Click the document icon by Replacement Serv. Parent topic: Forms

Complete the Jury Duty Form
Use the Jury Duty Template template to generate a note to excuse patients from jury duty. Jury Duty template Click the Forms link, and double-click Jury Duty in the list. From the Diagnosis grid, select a diagnosis. (Optional) From the Surgery grid, select a procedure. Enter the reasons to excuse by selecting the appropriate check box. Enter a time frame or duration in the From and To fields or For field, respectively. Select the Provider Signed check box, and select the provider from the list. Once all information is entered, select the Done check box. Your name and the current date fill the time stamp fields. Generate the document by clicking the Jury Duty document button. Complete the Travel Excuse Form
Use this template to generate a note to excuse or assist patients with travel issues or requirements. Travel Excuse template Click the Forms link, and double-click Travel from the list. In the Diagnosis grid, select a diagnosis. (Optional) In the Surgery grid, select a procedure. Select a reason for the excuse note. You can enter a reason not already listed, or add a comment, in the Misc field. Use the From/To or For fields to enter a time frame or duration. Select the Provider Signed check box and select the provider from the list. Once all information is entered, select the Done check box and generate the document by clicking the Air Travel document button. Parent topic: Forms

Complete the Return to Work Form
Use the Return to Work template to create a detailed return to work note/or document. The Return to Work template has four panels, each with several check boxes. Click the Forms link, and double-click Return to Work from the list. The Return to Work template opens. Return to Work template In the Diagnosis grid, select a diagnosis. In the Surgery grid, select a procedure. Enter in Return status, Work Restrictions, and General Restrictions by selecting check boxes in the appropriate panels. Select the Done check box to sign off. The Forms link opens a list of forms that enable you to do the following: Generate an attendant care request letter Generate excuse letters to restrict participation in, or excuse the patient from school, gym, return to work, jury duty, and, travel Generate letters for referrals, Medicare ABN, and replacement services Generate missed appointment and termination of care letters Forms link Forms picklist Creating Excuse Notes and Generic Letters You can create generic letters and excuse notes for a patient to describe the restrictions due to an injury. Generate Intake Note
You can generate the intake note for a patient using Generate Intake Note on the Intake template. Before you begin You must complete all the intake activities before generating the intake note. You can generate the intake note as a written summary of the encounter from the intake staff or medical assistant. Open NextGen® Enterprise EHR. Open the Intake template. Select Generate Intake Note.The system generates the intake note.
