Overview of the Histories Template

You can add, update, delete, and review a patient's history summary details. You can document a patient's social history, problem list, medical or surgical history, diagnostic study history, and family history on the Histories template. Patient's Problem List You can manage a patient's problems. Patient's Medical History The Medical/Surgical/Interim panel allows you to document the medical or surgical history of the patient. Patient's Social History You can document a patient's social history. Patient's Family History You can document the family history of the patient in the Family panel on the Histories template. Patient's Diagnostic Tests and Results You can document a patient's diagnostic tests and the associated results. Generate the Intake Note

You can generate the intake note for a patient using Generate Intake Note in theIntake and Histories template. The Generate Intake Note is used to generate the Intake Note as the written summary of the encounter from the intake staff or medical assistant. It is recommended that staff create the document after all of the intake activities are complete. It is not necessary to generate the note twice during a visit. When the intake staff completes documenting the patient information, the Intake Note is generated as the written summary of data collected up to this point in the encounter. The provider can also generate the Intake Note to obtain an overview of the collected information and then continue with the physical exam and assessment plan. Open the Intake or Histories template. Intake Note Generation

You can generate Intake notes on the Intake template after all the intake activities are complete. You can select Generate Intake Note on the Intake template or Intake Note on the Histories templates. You can create an Intake Note as the written summary of the encounter with the intake staff or medical assistant. Generate the Intake Note Document

After documenting the patient information, the intake staff generates the Intake Note as the written summary of data collected in the encounter. The provider might also generate the Intake Note to obtain an overview of the collected information and then continue with the history of present illness, physical exam, assessment, and plan. Generate Information from Document Library

You can generate patient information using the documents listed on the Documents and Letters template. Open NextGen® Enterprise EHR. Open the Intake template. Select the Document Library link.The Documents and Letters template opens. Note: You can go to the Document and Letters template using the Document Library link on the navigation bar. Clinical Narratives for Clinical Note Types You can manage various clinical narratives. Add Clinical Narratives You can add clinical narratives for clinical note types for the current encounter. Manage Clinical Narratives You can review, update, remove, or restore clinical narratives using the Clinical Narratives template. Generate Notes Associated with Encounters

You can generate notes associated with an encounter on the SOAP template. Open NextGen® Enterprise EHR. On the Intake or SOAP template, select Generate Notes in the Navigation Buttons section. Select the note to generate from the available list. Select OK.
