Complete the Replacement Services Form

Use the Replacement Services template to generate a note for any required replacement services.
Replacement Services template

  1. Click the Forms link, and double-click Replacement Services.
  2. In the Diagnosis grid, select a diagnosis.
  3. In the Surgery grid, select a procedure.
  4. Select the desired replacement services. Use the Misc. field to enter any other types of care.
  5. Enter the date range and hours.
  6. Select the Signed check box and enter the Provider.
  7. Once all information is entered, select Done.
  8. Click the document icon by Replacement Serv.
