Restrictions	To enter the restrictions for the patient in gym and sports activities.
Generic Letter	To document the details to create a generic letter for any other occasions.

Complete the Jury Duty Form

Use the Jury Duty Template template to generate a note to excuse patients from jury duty. Jury Duty template Click the Forms link, and double-click Jury Duty in the list. From the Diagnosis grid, select a diagnosis. (Optional) From the Surgery grid, select a procedure. Enter the reasons to excuse by selecting the appropriate check box. Enter a time frame or duration in the From and To fields or For field, respectively. Select the Provider Signed check box, and select the provider from the list. Once all information is entered, select the Done check box. Your name and the current date fill the time stamp fields. Generate the document by clicking the Jury Duty document button. Complete the Travel Excuse Form

Use this template to generate a note to excuse or assist patients with travel issues or requirements. Travel Excuse template Click the Forms link, and double-click Travel from the list. In the Diagnosis grid, select a diagnosis. (Optional) In the Surgery grid, select a procedure. Select a reason for the excuse note. You can enter a reason not already listed, or add a comment, in the Misc field. Use the From/To or For fields to enter a time frame or duration. Select the Provider Signed check box and select the provider from the list. Once all information is entered, select the Done check box and generate the document by clicking the Air Travel document button. Parent topic: Forms

Create Excuse Notes

You can create a patient's excuse notes for school and work. Open the Forms template. Excuse panel on the Forms template Select the Pt Seen check box. The calendar window opens to select the date of the appointment. Select the date on the calendar window, and click OK. Enter the time of the appointment as follows: To enter the current time, click the Clock icon icon. To enter a specific time, click the Down arrow icon icon, enter the time in the time entry window, and then click OK. In the Excuse panel, do the following: Select the appropriate options for excuse. Type any comments for excuse, if needed. Click the Provider field, select the name of the provider from the providers window, and then click OK. Note: By default, the name of the rendering provider is displayed in this field. Select the Done check box. Create Restrictions Notes

You can create a patient's restrictions notes for gym and sports activities. Open the Forms template. Restrictions panel on the Forms template Select the Pt Seen check box. A window opens for selecting the date of the appointment. Select the date on the calendar window, and click OK. To enter the time of the appointment, do one of the following: To enter the current time, click the Clock icon icon. To enter a specific time, click the Down arrow icon icon, enter the time in the time entry window, and then click OK. In the Restrictions panel, do the following: In the No participation in section, select the activity that patient cannot participate in. A window opens for selecting the duration. Enter the duration of the patient's inactivity, and click OK. Note: You can also indicate if the patient cannot participate in the selected activities until cleared by provider or next appointment. Create Generic Letters

On the Forms template, expand the Generic Letter panel. Generic Letter panel on the Forms template In the Create Letter section, do the following: Select the option for the intended recipient of the letter. Note: If you select the To Provider check box, a window opens for the selection of the provider name. You can select the desired provider name. If you select the Thank you for check box, you can type a thank you note in the blank field next to the check box. Select the appropriate option for greeting text, and modify the greeting text, if needed. In the Body of letter field, type the text of the letter. To document the appointments, do the following: Select the Appointment Changes check box. Click the Appt. Type field, and select the appointment type from the list. Click the Appt. with field, select the provider's name from the providers window, and then click OK. Click the Appt.
