Filter the Patient's Clinical Problems

The Clinical Problems section of the Assessment/Plan - OPH template lists tracked problems and chronic conditions for the patient. clinical problems section On the Assessment/Plan - OPH template, expand the A/I/P Details panel and ensure that the DX HX/My Favourites option is selected. Note: The DX HX/My Favourites option is selected by default. For more information on changing the default option, go to NextGen Healthcare Success Community and download the latest Getting Started User Guide for NextGen® Adaptive Content Engine. Select the Chronic check box to view the patient's chronic conditions. Select the Tracked check box to view the patient's tracked problems. To learn how to mark a problem as chronic or tracked, go to Add Problem to the Problem List. Select the No Active check box to view the patient's problems that are no longer active. Patient's Problem List

You can manage a patient's problems. You can perform the following actions in the Problem List panel. Record, modify, and retrieve a patient's clinical problem list electronically to meet Quality Reporting requirements Document the patient's chronic conditions and ongoing clinical problems View problems from past encounters Add problem details Problem List You can also indicate if a patient's documented problem is completed and reviewed, and if there is any active problems. You can select the Include Resolved check box to include the resolved problems. Open Problem List You can access the problem list to document and view a patient's problem history. Filter the Patient's Diagnosis History

The Dx History section of the Assessment/Plan - OPH template lists a recent history of diagnoses for the patient. dx history window In the Dx History section, select one of the following options to filter the patient's diagnosis. Patient's Problem list

You can do the following in the Problem List panel: Electronically record, modify, and retrieve a patient's clinical problem list to meet Quality Reporting requirements Document a patient's chronic conditions and ongoing clinical problems. View problems from past encounters Update problem details To indicate that the review of the patient's documented problem is complete, the provider must select the Reviewed check box. Remove Chronic Conditions from the Reason for Visit

Diagnostic studies added to the history of present illness in the Reason for Visit panel appear in the grid with Studies Reviewed in the Reason for Visit column. On the Intake- OPH template, expand the Reason for Visit panel. Select the Chronic Conditions record from the grid. The Remove button will been enabled. Click Remove. The Chronic Conditions record no longer appears in the grid. Note: Removing the record from the Reason for Visit grid does not remove it from the chronic conditions grid. Select Diagnosis Panel

In the Select Diagnosis panel of the Select Tests tab, you can select one or more diagnoses to be associated with the tests ordered. The list of diagnoses is pulled from the patient's chart.
