Record Color Vision Findings

Record the Amsler Grid Test Results
In the Amsler Grid panel, enter the Amsler findings in the following fields: OD OS OU If applicable, select the result for the OU field. Note: Findings entered in the OU field, fill the OD and OS findings. Click the Draw Amsler Grid link to open an image and draw the patient's test results. Parent topic: Low Vision Exam

Document Corneal Light Reflex Test Results

Enter Cover Test Findings
Cover test findings for both Without Correction and With Correction have the same workflow. For Dist, click the Prism1 field and select a value from the list. The picklist for the Base1 field opens. Select a value for Base1 from the list. Click the Prism2 field and select a value from the list. The picklist for the Base2 field opens. Select a value for Base2 from the list. Repeat the steps above for the Near fields. Enter the findings in the sc field for Without Correction or the cc in the With Correction. Enter the Fixation/Pref for Dist and Near and then enter Head Posture. Enter additional Comments as needed. Parent topic: Low Vision Exam

Document Contrast Sensitivity Test Findings

The provider can document the exam for a low vision visit on the LV Exam - OPT template. You can review the testing performed by the technician and perform additional exams as needed.
