Record Color Vision Findings

Document Corneal Light Reflex Test Results

Record the Amsler Grid Test Results
In the Amsler Grid panel, enter the Amsler findings in the following fields: OD OS OU If applicable, select the result for the OU field. Note: Findings entered in the OU field, fill the OD and OS findings. Click the Draw Amsler Grid link to open an image and draw the patient's test results. Parent topic: Low Vision Exam

Enter Visual Acuity Findings
To document visual acuity, you can select OD and OS values and modifiers, as appropriate. The system moves you to the next field as you document each parameter. For example, after you document the OD Dva sc and modifier, the picklist opens for OS Dva sc. In the Visual Acuity panel, click Add. The Oph Eyetech template opens. Select the OD value for Dva sc. The modifier pick list opens. Select a modifier from the list or click the blank row at the top of the list if no value is needed. The system moves to the OS value for Dva sc. Repeat the steps above for each parameter. Enter the Correction Type and Vision Method. Enter the Comments for OD, OS, or OU. Click Add. The documented findings are uploaded to the corresponding visual acuity grids Edit Visual Acuity Findings Remove Visual Acuity Findings You can only remove the visual acuity findings from the current encounter. Document an External Exam
On the LV Exam - OPT template, expand the Physical Exam panel. To copy the last documented external exam findings to the current exam fields, select the Copy last exam check box. Note: You must have access to use copy-forward. If all external exam findings are normal, select the appropriate External WNL check box for each eye. Exam parameter fields fill with normal default finding values. Enter the pupil size for Dim and Bright light for each eye. Document OD and OS findings as follows: Click Findings field, and select a value from the list. Abnormal findings appear in red font. Click the Modifier field, and select a value from the list. If OD and OS findings for a parameter are the same for both the eyes, click the arrows between the OD and OS fields to copy an entry from one eye to the other.
