Document an External Exam
On the LV Exam - OPT template, expand the Physical Exam panel. To copy the last documented external exam findings to the current exam fields, select the Copy last exam check box. Note: You must have access to use copy-forward. If all external exam findings are normal, select the appropriate External WNL check box for each eye. Exam parameter fields fill with normal default finding values. Enter the pupil size for Dim and Bright light for each eye. Document OD and OS findings as follows: Click Findings field, and select a value from the list. Abnormal findings appear in red font. Click the Modifier field, and select a value from the list. If OD and OS findings for a parameter are the same for both the eyes, click the arrows between the OD and OS fields to copy an entry from one eye to the other. Repeat the steps above for each exam parameter you want to document. To save the documented values as the default values, click the Quick Save, and enter a name for the template. This feature en

Document Contrast Sensitivity Test Findings
In the Contrast Sensitivity Test panel, select the Test Type used to measure contrast sensitivity. Enter the findings for the following: OD OS OU If applicable, select one of the following: 25% 10% 5% 2.5% 1.25% Enter additional information in the Comments field. Parent topic: Low Vision Exam

Low Vision Exam
The provider can document the exam for a low vision visit on the LV Exam - OPT template. You can review the testing performed by the technician and perform additional exams as needed. Enter Visual Acuity Findings Document an External Exam You can document the external exam of an eye in the External Exam panel. Document a Slit Lamp Exam Document a Posterior Exam Enter Cover Test Findings Enter Nystagmus Findings You can record findings associated with nystagmus in the Nystagmus panel. Record Color Vision Findings You can record color vision deficiencies in the Color Vision panel. You can document and create a comprehensive, individual rehab plan for the low vision patient based on the patient's visual abilities, goals, and devices on the LV Devices - OPT template. Goals and devices can be monitored and updated in future encounters. Create Goals for the Low Vision Patient You can create custom goals for the low vision patient on the LV Devices - OPT template. Record Low Vision Devices You can record the recommended low vision devices, acuity achieved, and skills for the patient on the LV Devices - OPT template.
