Enter Assessments for Today's Visit
Assessments can be added to Today's Assessments in several ways, including by selecting from the patient's clinical problems or diagnosis history, the provider's list of favorite assessments, the Common Assessments template, or the diagnosis search window. New assessments can be added to various lists for the patient or the provider's My Favorites list, and they can be sorted to display in a preferred order. If a technician is acting as a scribe for the provider, select a name from the Scribe field. Review Today's Concern/Reason For Visit. Select an assessment from the grids in the following areas to add it to the Today's Assessment grid. Clinical Problems Dx History list My Favorites list To select an assessment from the Common assessment template, click the Common Assessment link. To search for an assessment, click the Diagnosis Lookup link. Document an Impression. Document a Plan by typing in the field or click the following links. Add a Previously Documented Impression to the Today's Impression/Plan Summary Grid
Before you begin For proper functioning of UI elements, it is recommended to download and install Windings 2 font on \Windows\Fonts folder. The Previous Impression/Plan grid of the Assessment/Plan - OPH template lists previously documented impressions or plans and you can add these records to the Today's Impression/Plan Summary grid. On the Assessment/Plan - OPHtemplate, expand the A/I/P Details panel, and then select the Previous Impression/Plan option. Note: The DX HX/My Favorites option is selected by default. For more information on changing the default option to Previous Impression/Plan, go to NextGen Healthcare Success Community and download the latest Ophthalmology Setup Guide forNextGen®Adaptive Content Engine. The Previous Impression/Plan grid opens. Select the records from the Previous Impression/Plan grid, and then click Add to Grid. Note: General plan orders are not added to Today's impression/Plan Summary grid. The selected records appear in the Today’s Impression/Plan Sum

Update Records in Today's Impression/Plan Summary
The records in the Today's Impression/Plan Summary grid can be edited and updated. Open the Assessment/Plan - OPH template, expand the A/I/P Details panel, and then ensure that the DX HX/My Favourites option is selected.a/i/p details panel Double-click a record from the Today's Impression/Plan Summary.The Oph - Today's Impression/Plan Orders window opens. Make the necessary changes to the record, click Update and then click Close.The updated information appears in the Today's Impression/Plan Summary grid. Parent topic: Document the Impression

Documenting an Impression and Plan
Providers can document their assessments, impressions, plans, return to clinic orders, and enter internal physician notes on the Assessment/Plan tab. Enter Assessments for Today's Visit Ophthalmic providers can enter the assessment, impression, and plan on the Assessment Plan - OPH template. Review Today's Assessments
Ophthalmic providers can review previously documented diagnoses/assessments that were selected for the patient for this encounter listed in the Today's Assessments grid in the Diagnosis panel on the Finalize - OPH template. CAUTION: A diagnosis added or modified on the Finalize - OPH template does not appear in the Impression/Plan Summary grid. However, it appears in the Diagnosis History and in the Chart Note. Best practices recommend that the provider add and update assessment on the Assessment/Plan - OPH template so that they are listed in the Impression/Plan grids and can be associated with plan details. impression/plan grid Parent topic: Finalize Tab for Ophthalmic Providers

Document a Postoperative Exam Impression and Plan
You can document an impression, plan, and return to clinic details in the Impression and Plan section of the Postoperative Exam – OPH template. Select the procedure to document by selecting the option Status Post Procedure 1 or Status Post Procedure 2. (Each procedure is documented separately.) Click the Dx Description field and select a diagnosis from the diagnosis search tool. The code associated with the diagnosis appears in the Code field. If applicable, click the Scribe field and select the scribe. To document any postoperative complications necessitating additional surgery within 30 days after cataract surgery, click the Post-op Complications link to open the Cataract Post-Op Complications template. Document an impression by selecting check boxes for any post-op impression details that apply. A paragraph builds in the large Impression text field as selections are made. After selecting check boxes, additional impression details can be typed in the text field using free text and My
